| Objective:To compare the clinical effect of lesser trochanter reduced and fixed or not in the treatment of unstable f intertrochanter fracture of femur with DHS in order to offer basis on choosing the proper treatment method in clinic.Method:To choose 38 cases of unstable intertrochanteric fracture of femur treated in Orthopedics of the People's Hospital of Jieshou from May 2008 to October 2009. They all tallied with the criterion that should be subsumed or eliminated. We divided them into two groups randomly with the random number table .Then one group of 19 cases was treated with DHS adding lesser trochanter reduced and fixed,the other group with DHS only. To compare and analyze the clinical effect of two different methods in the common conditions, operative procedure, the complications ,fracture healing and the functional evaluation of hip joint. The statistical analysis was performed by SPSS 13.0.Result: All the patients were followed. There were no significant difference between two groups the volume of drainage after operation and the complications(P>0.05) .The operation time were 75.42±6.81 minutes for the group of lesser trochanter reduced and fixed, 66.58±5.66minutes for the group of lesser trochanter not reduced and fixed. There were significant difference between two groups(P < 0.05). The blood loss during operation were 512.10±86.93 milliliters for the group of lesser trochanter reduced and fixed, 346.84±68.64 milliliters the group of lesser trochanter not reduced and fixed. The volume of blood transfusion during operation were 226.32±128.42 milliliters for the group of lesser trochanter reduced and fixed, 110.53±80.93 milliliters the group of lesser trochanter not reduced and fixed. There were significant difference between two groups(P<0.05). According to the Harris's hip functional standard score, the scores were 89.46±2.82 for the group of lesser trochanter reduced and fixed,84.23±4.46 for the group of lesser trochanter not reduced and fixed. There were significant difference between two groups(P<0.05).Conclusion:1. There were no statistical difference in two groups in the volume of drainage after operation and the complications.2. The group of lesser trochanter reduced and fixed is longer in the operation time, more in the blood loss and the volume of blood transfusion during operation, but better in the function of hip joint after operation than others.3. It is very important to reduce and fix lesser trochanter for unstable femoral intertrochanteric fracture treated with DHS. |
