OBJECTIVE According to selective coronary angiography as the "gold standard" of detecting coronary artery disease,the study is been used to assess the diagnostie value of 12-leadHolterST segment and the maximum ST/HR.METHODS In this study, the 117 patients were examed by CAD during January 2005 and January 2010 at the First Affiliated Hospital of Shanxi Medical University, Department of Cardiology.And all patients underwent 12-lead Holter and echocardiography, and with 12 lead Holter interval was less than 2 weeks.To statistic on the branch of the occurrence of coronary artery disease, statistic the incidence rate of ECG abnormal ST-segnent and the maximum ST/HR in Patients with the different extent of coronary artery disease. To analyzed the sensitivity and specificity of 12 lead Holter diagnosis of coronary heart disease and its relationship with the degree of coronary artery disease. To statistic the occurrence rate of abnormal ST-segment and the maximum ST/HR in othe diseases except coronary artery disease, the relationship between the changes of cardic structure and the abnomal ECG, for further accoutting for the importance of 12 lead Holter in diagnosin of coronary artery disease, in order to provide the value of clinical diagnosis.RESULTS 117 patients were selected with strict inclusion criteria and exclusion criteria, including 71 male patients,46 female patients. Of which 86 were confirmed coronary heart disease,31 non-CAD group.There are 79 people in 12-lead Holter ST segment shift positive group,38 people in negative group, the diagnosis with a sensitivity of 79.07%, specificity was only 64.52%, for single vessel disease had a sensitivity of 58.62%, on multi-vessel disease had a sensitivity of 89.47%. There are 71 people in the maximum ST/HR positive group and 46 in negative group, his diagnosis with a sensitivity 88.37%, specificity was 93.55%, single vessel disease had a sensitivity of 75.86% on multi-vessel disease had a sensitivity of 94.74%. There are 11 CAD patients in non-ST-positive group,4 cases of left ventricular hypertrophy, left atrial enlargement with left ventricular hypertrophy in 5 cases,1 case of left atrial enlargement. Non-CAD group and the maximum ST/HR were positive in 2 cases of left atrial enlargement with left ventricular hypertrophy.CONCLUSIONS Multiple coronary heart disease in elderly populations, and more men than women; 12-lead Holter ST segment and the maximum ST/HR are the better index of the diagnosis of coronary artery disease, which the ST/HR diagnostic value of greater; with the severity of coronary artery disease,12-lead Holter positive detection rate increased. |
