| Objective To evaluate the diagnostic value of 12-lead Holter in the coronary heart disease(CHD). Methods Eighty senven patients were examined by CAG and 12-lead Holter. The duration of the both method was less than one week. Base on coronary angiography (CAG), ST-segment dataof 12- lead Holter were compared with CAG. Results 1. The sensitivity and specificity of 12 -lead Holter in the diagnosis of CAD were 85. 2%, 81. 5% respectively. The positive rate of 12-lead Holter in the single vessel CAD group and multi-vessel CAD group is 80.6%, 93.1% respectively. There was no significant difference between the multi-vessel CAD group and the single vessel CAD group (P>0. 05). 2. In the single vessel group, the range of ST-segment depression and the continued time in ≥75% coronary stenosis group were more than the two index in 50%-74% coronary stenosis group. There was significant difference between the two group (P<0. 05) . 3. In the single vessel group , there are 16 CHD reselted from LAD, 8 CHD from RCA and 1CHD from LCX. Among 16 CHD, there are 5 ST segment depression at V4-V6 and 11 at II III avFV5V6 . Among 8 CHD , there are 1 ST segment depression at II III avF and 7 at II III avF V5V6. The ST segment depression of 1 LCX CHD is at V5V6. 4. In the 30 patients of CHD who were confirmed by CAG, the positive rate of 12- lead Holter, 3- lead Holter and ECG in the diagnosis of CHD were 86. 7% , 76. 7%, 46. 7% respectively.There is an apparent difference between them (P<0. 05). Conclusion 1. The sensitivity, specificity of 12 -lead Holter in the diagnostic of CAD were higher, so 12-lead Holter test is an effective non-invasive method in the diagnosis of CAD. The shape of ST-segment is helpful for the diagnosis of the kinds of CHD. The range of ST-segment depression and the continued time index could forcast and judge the degree of coronary stenosis to a greater or lesser extent. 2. 12-lead Holter is better than 3-lead holter and ECG in the positive rate of CHD. |
