Objective:To guide the clinician to select the reasonable operation occasion and treatment schema for Anterior Cruciate Ligament fragmentation combined Medial Collateral Ligament injuries of the Knee, a study has been made by comparing the clinical effectiveness of different phases and treatment schemas for this kind of injury.Methods:From 2006.03 to 2009.03,154 ACL/MCL associated injury patients are treated by our hospital,in which 44 are MCL I or II degree injury with ACL fragmentation. Expectant treatment is used to treat MCL,while operation is performed to treat ACL.There're 110 MCLⅢdegree injury associated with ACL fragmentation.In the MCLⅢdegree associated injuries,34 patients were performed ACL and MCL recons-truction at acute phase(A group),32 were only used ACL recons-truction at delayed chronic phase(B group).44 were performed ACL and MCL delayed reconstruction(C group). A comparison is made between the preoperative and postoperative effectiveness of the 44 MCLⅠorⅡdegree injury associated with ACL fragmentation patients;then the postoperative effectiveness of the MCLⅢdegree injury with ACL fragmentation patients among A,B,and C group. Statistical analysis was proceed.Results:In MCLⅠorⅡdegree injury with ACL fragmentation group, the average preoperative Lysholm score is 35.18±6.1,the average postoperative of following up score is 94.15±5.7.the preoperative and postoperative comparison have significant deviation.(P<0.01) videlicet the postoperative effectiveness is more obvious. In the comparison of the different treatment schema of the MCLⅢdegree injury associated with ACL fragmentation group, the average postoperative of following up Lysholm scores are respectively 92.25±5.68,93.15±5.31,91.38±6.18 in the A,B,and C group.There is no significant deviation.,videlicet the postoper-ative effectiveness among the three group is equivalent.Conclusions:According to the different stage treatment of anterior cruciate ligament and medial collateral ligament of injuries of the Knee,the clinical research result indicates that, for the ACL fragmentation associated withⅠorⅡdegree MCL injury, executing conservation treatment to acquire MCL union or part union,then reconstructing the ACL can acquire a positive effect. For the ACL fragmentation associated with III degree MCL injury reconstructing the ACL and MCL at the same time in the acute phase,only reconstructing the ACL in the delayed chronic phase,reconstructing the ACL before the MCL reconstruction, there is no significant deviation.
