ObjectiveIn recent years, overweight and obesity has become a global health problem.The rising rate of obesity is a major public health concern in the world including China.Pregnancy and delivery in case of overweight women require special care. Several studies have reported that many adverse pregnancy outcomes such as pre-eclampsia, gestational diabetes, cesarean delivery, delivery of large-for-gestational age infants, stillbirth and congenital anomaly are associated with increased pre-pregnancy body mass index and weight gain during pregnancy.But most of them are from western countries.There are many differences in the characteristic of populations, culture and medical condition, In China,the exact incidence of obesity in pregnant women is not known and the effect of obesity on Chinese might be not clear, so the objective of this study is to investigate the prevalence of pregnancy obesity and pregnancy outcomes of obese women in Shenyang area.Materials and MethodsA total of 4326 Singleton nulliparous pregnancies delivered in three hospitals of Shenyang from 2008 to 2009 were enrolled.We calculated the prevalence of obesity and estimated the adverse pregnancy outcomes of obese pregnancies.Results5.62% of the pregnant women were pre-pregnancy obesity.50.12% of women gained more than recommended amount of weight during pregnancy. Compared to the normal weight group of pre-pregnancy, pre-pregnancy obese women had increased the risk of gestational hypertension (RR 5.32;95%CI3.42-8.45), gestational diabetes mellitus (RR5.12; 95%CI 3.03-7.11), cesarean section (RR 2.61;95%CI 1.89-3.52), premature rupture of membrane (RR1.72; 95%CI 1.11-2.71), premature delivery (RR1.85; 95%CI 1.05-3.26) and macrosomia (RR2.17; 95%CI 1.32-3.68). Excess gestational weight gain increased the risk of gestational hypertension(RR1.93; 95%CI 1.46-2.55), gestational diabetes mellitus(RR1.52; 95%CI 1.46-2.55), cesarean section (RR1.40; 95%CI 1.18-1.67)and macrosomia(RR2.01; 95%CI 1.47-2.77),but decreased the risk of having low birth weight baby(RR0.52; 95%CI 0.36-0.76).ConclusionsThe situation of pregnancy obesity is serious. Both pre-pregnancy obesity and excess gestational weight gain can significantly increase the risks of adverse pregnancy complications.
