Objective:To analysis the influencing factor of the extensive burn patients with stress hyperglycemia.To study the incidence of the extensive burn patients with stress hyperglycemia or diabetes. To evaluate the differential diagnosis, treatment and prognosis of the extensive burn patients with diabetes mellitus complicated by stress hyperglycemia.Methods:We collected the clinic data of 458 extensive burn patients who was treated in the No 1 hospital of Guangxi medical university from July 2002 to June 2009. We used the meter to test the fasting plasma glucose, and selected 129 cases whose fasting plasma glucose was elevating. Then we asked the history of diabetes, and divided these patients into complicated stress hyperglycemia group (105 cases) and diabetes group (24 cases). We compared the complication of stress hyperglycemia in patients with different age groups and different incidence of burn area to analyze the complicated stress factors of stress hyperglycemia. Furthermore, we gave a different treatment in the either group, and obtained the differential diagnosis criteria, treatment and prognosisResults:1.There was no significant correlation between the incidence of the extensive burn patients with stress hyperglycemia and the age. (P> 0.05) 2. There was significant correlation between the incidence of the extensive burn patients with stress hyperglycemia and the area of burn. 3. The hyperglycemia time of the extensive burn patients complicated with stress hyperglycemia is shorter than the extensive burn patients with diabetes.4. The fluctuations in blood glucose levels is larger than diabetic patients. They can not use the insulin to control blood sugar..Conclusion:1. The incidence of the extensive burn patients with stress hyperglycemia complicated by the area of burn. The greater is the area, the higher is the incidence of stress hyperglycemia. However, there is no significant correlation between the incidence and age.2. The change of blood sugar of the extensive burn patients complicated with stress hyperglycemia is different from the diabetic patients. Most of them can not use the insulin, and generally does not require application of oral anti—diabetic drugs. The prognosis of then is better than the extensive burn patients with diabetes.3. Most extensive burn patients complicated with the diabetes mellitus need the insulin or the oral anti—diabetic drugs to reduce the blood sugar. They have a lot of complications. And the cure rate of them is lower. However, they still need to monitor blood sugar cured and use of drugs. And some of them will possible relapse. |
