| Objective: To investigate the influence of diabetes mellitus(DM) and stress hyperglycemia (SHG) on the recent prognosis of the patients with acute myocardial infarction(AMI)Methods: 240 continuously inpatient with AMI were analyzed and divided into 3 groups on the basis of previous diabetes history, fasting plasma blood glucose level and the test of glucose intolerance next day in the Morning. AMI with normal blood glucose (n=90); AMI with stress hyperglycemia (SHG)(n=88) and AMI with diabetes mellitus(DM) (n=62) group. recorded the complications of during hospitalization and 3 months after the situation.Results: The incidence of worse prognostic events, such as asymptomatic myocardial infarction, arrhythmia, cardiogenic shock and heart failure were increased in patients with overt diabetes and stress hyperglycemia compared with AMI cases without DM or stress hyperglycemia and had statistical significance. Moreover, the incidences of worse prognostic events were lower in well-controlled group than in poorly controlled group and the value was also statistically significant.Conclusion: Diabetes and SHG are associated with increased prevalence and severity of arrhythmia, cardiogenic shock; heart failure in AMI patients. Aggressive blood glucose control may decrease the incidence of complication of AMI and increase the recent survival rate. |
