| objective:Explore the relationship between the type of gastric pit by magnifying chromoendoscope and pathology ,and the significance of Runx3 in gastric mucosal lesion.Methods:97Patients were included in the study,they all had upper digestive symptom and were examined by the same senior doctor using magnifying endoscopy Real-time diagnosis was made during the examination,Biopsy were taken for histopathology and detected the expression of antioncogene runx3 and smad4.56 mans,41 women,average age 39.3 years. The pit patterns of gastric mucosa were classified typeA(round spot),B(short rod),C(branched),D(reticular),E(villus)and F(irregular).we analyzed the relationship between pit patterns of gastric mucosa and histopathology,tested the expression of antioncogene runx3 and smad4 in Chronic superficial gastritis,Intestinal metaplasia,atypical hyperplasia and stomach cancer.Result:(1) relationship between the type of gastric pit and pathology.33 are type C,,9are typeD,11are typeE,14are type F,there are different between every two type of gastric pit and pathology.The most of type of gastric pit in mild superficial gastritis and intestinal metaplasia are type C,while the most of type of gastric pit in moderate superficial gastritis and ulcer are type D,Statistical significance can be found in type C,D and pathology(P<0.01).The most of type of gastric pit in atypical hyperplasia are type E,most of cancer are type F,Statistical significance can be found in typeE,F and pathology(P<0.01).There are statistical significance in every two type and pathology.relationship between the type of gastric pit and Pathology is obvious by Spearman correlation analysis,r=0.56,P<0.01.(2) The relationship between the type of gastric pit and expression of Smad4,there are expression of Smad4 in type of C,D,E,F,it is more higher than the type of D,E,F, the expression of Smad4 are reducing gradually from type C to type F,Statistical significance can be detected in every two group(P<0.05).(3)The relationship between pathology and expression of Smad4,the expression in superficial gastritis are higher obviously than Intestinal metaplasia, atypical hyperplasia, gastric cancer(P<0.05).expression of Smad4 in bosh atypical hyperplasia and gastric cancer are slow,Statistical significance cannot be detected in this two group(P>0.05).The expression of Smad4 in atypical hyperplasia are higher obviously than gastric cancer,there are Statistical significance in this two grope(P<0.01),The expression of Smad4 in Intestinal metaplasia are higher obviously than atypical hyperplasia,there are Statistical significance in this two grop(eP<0.05),(4)The relationship between the type of gastric pit and expression of Runx3,it is more higher than the type of D,E,F, the expression of Runx3 are reducing gradually from type C to type F,Statistical significance can be detected in every two group(P<0.05).(5)The relationship between pathology and expression of Runx3,the expression in superficial gastritis are higher obviously than Intestinal metaplasia, atypical hyperplasia, gastric cancer ( P<0.05 ) .expression of Runx3 in bosh atypical hyperplasia and gastric cancer are slow,Statistical significance cannot be detected in this two group(P>0.05).The expression of Runx3 in atypical hyperplasia are higher obviously than gastric cancer,there are Statistical significance in this two grope(P<0.05),The expression of Runx3 in Intestinal metaplasia are higher obviously than atypical hyperplasia,there are Statistical significance in this two grop(eP<0.05),(6) relationship between the type of Runx3 and Smad4 is obvious by Spearman correlation analysis,r=0.86 P<0.01Conclusions:1.Gastric inflammation was increasing when gastric pit from C to D in nonatrophic gastritis;2.The type of gastric pit C, D, E, F were found separately in nonatrophic gastritis, mild intestinal metaplasia atypical hyperplasia, gastric cancer;3.The expression of Runx3,smad4 was reducing in nonatrophic gastritis, mild intestinal metaplasia,atypical hyperplasia, gastric cancer;4.The expression of Runx3,smad4 was reducing in type C,type D,type E,type F;5.Positive correlation of the expression of Runx3 and smad4 was found in nonatrophic gastritis, mild intestinal metaplasia,atypical hyperplasia, gastric cancer;6.Precancerous lesions and gastric cancer will be more possible in this type of gastric pit D,E,F and low expression of antioncogene runx3 and smad4. |
