Objective: Summed up subtemporal lateral approach microsurgical treatment of middle cranial fossa and petroclival region tumor efficacy and clinical application experience, as well as experience.(Front temproal-zygo- matic arch-subtempora, Temporal suboccipital-tentorial transprtrosal app- roach,subtemporal-anterior transprtrosal approach).Approach to explore the scope of the difficult surgical approach and the superiority of surgical app- roach, in order to increase the level of surgical operations and surgical results, how to reduce complications after surgery may make the preventive measures, in order to provide valuable clinical reference.Methods: Clear of the preoperative diagnosis for the middle cranial fossa and petroclival region at the end of the seven cases of tumor patients, the location of choice for tumors of the infratemporal different lateral approach, Field of vision so that the expansion of the operation, close to the tumor, the tumor has fully exposed the middle cranial fossa and petroclival region and the relationship between the relevant anatomy. Under the microscope to observe the surgical field exposure and tumor excision, the nerves and blood vessels of the protection, recovery of postoperative follow-up. Discussion at the end of the middle cranial fossa and petroclival region tumor through lateral infratemporal approach microsurgery of indications, complications, analysis of the impact of selection by the lateral infratemporal approach microsurgery of the relevant factors.Results: 7 cases of this group of six cases of patients with total resection by endoscopic imaging confirmed the total resection rate of 85.7%, a case of near-total resection. The amount of one case of postoperative shallow pattern, facial numbness in one case, a case of temporal lobe edema, and subcutaneous fluid oculomotor nerve palsy. Postoperative follow-up of October-December, the disappearance of post-operative complications, tumor recurrence and no sign of growth to expand.Conclusion : Lateral subtemporal approach to saddle the next pre-pontine space, petroclival region, ventral brain stem and cerebellar lesions as the center screen with a very important clinical significance, we have adopted this approach to obtain a good surgical results trauma is relatively small, much shorter operative time, length of stay of patients decreased significantly.With minimal brain damage and minimal surgical complications, can achieve the maximum extent of tumor resection, to avoid the surrounding blood vessels, nerve damage, the preservation of the brain tissue of normal physiological function. Therefore, we believe that the lateral infratemporal approach to solve the above-mentioned regional disease the best surgical approach options.
