Objective: The Metabolic syndrome(MS) consists of a combination of metabolic abnormalities, including abdominal obesity, high blood pressure,dyslipidemia and impaired glucosemetabolism.The components of MS were risk factors of heart disease and the cerebrovascular atherosclerosis disease[4-6]. In recent years, the prevalence of the metabolic syndrome has gradually increased in all populations worldwide with lifestyle change [1-3].human cytomegalovirus were the DNA virus with double strands, Since Fabbricant established the model of atherosclerosis with birds herper virus on turkey, studying the association between human cytomegalovirus and atherosclerosis was popular in world . Many studies have indicated that the human cytomegalovirus was related with artherosclerosis[7-10]and DM, and the association between MS and HCMV infection was rarely reported . Based on above, the study observed the prevelence of human cytomegalovirus in MS patients and to study further the pathogenesis of MS.Methods: 1744 health population for physical examination from May 2008 to December 2008 were enrolled. The patients with organ transplantation,taking immunosuppressed medicine for long-term,HIV infection,bone marrow disturbance with haematogenesis,upper airway and gastrointestinal tract infection,a variety of chronic and wasting disease,administered antibacterial and antivirus medicine for recent two weeks were excluded .Transcranical Doppler (TCD) were done in the MS individual to screen atherosclerosis occlusive diseases . Information obtained from all the subjects including demographic features, medical history and laboratory investigation was registered .The vascular risk factor such as hypertension, diabetes mellitus and hyperlipemia in our study was diagnosed respectively according to corresponding diagnosis criterion [11-13]. International Diabetes Federation (IDF) diagnosis standard of MS [14] and HCMV IgG(+)standard according to instruction were utilized .All studied individual was divided into two groups as HCMV negtive and HCMV positive respectively . We compared the mobidity of MS in two groups and evaluated the relationship of HCMV infection and MS,MS individual components. Further ,the MS group was divided into two groups according to HCMV result as HCMV negative and positive too .the mobidity of AS in two [ HCMV(-) group,HCMV(+) group] groups was compared to evaluate the relationship of HCMV infection and artherosclerosis.Result: 1760 patients initially were enrolled, 16 individual with poor acoustic windows were excluded . Finally,only 1744 subjects were included. Among of them , men were 1035,women were 709.The MS morbidity in HCMV positive group was 35.19%, which is higher than that of without HCMV infection group(16.70%), there was a significant difference between two groups (χ2=77.661,P<0.0001).After Multiple logistic regression analysis for sex, age, smoking,drinking, The Odds ratio(OR) of MS associated with cytomegalovirus infection was 2.106 ( 95%CI,1.816~2.442,P<0.0001). After adjusting MS all components , cytomegalovirus infection was also significantly associated with MS, Odds ratio(OR) was 1.598(95%CI,1.216~1.942,P<0.0001). Moreover ,Cytomegalovirus infection increase the risk of MS components too, corresponding OR of hyperglycaemia,hypertriglyceridemia ,hypertension ,hypercholesteremia , higher LDL was 3.482,( 95%CI 2.630~4.611 P<0.0001),2.649(95%CI2.111~3.325,P<0.0001),1.425(95%CI1.301~1.561,P<0.0001), 1.396(95%CI 1.223~1.593,P<0.0001), 1.501(95%CI , 1.061~2.213 , P=0.0216 ) ,respectively,and cytomegalovirus infection didn't increase the risk of abdominal obesity(OR 1.085,95%CI,0.855~1.379,P=0.501)and low HDL (0.984,95%CI,0.875~1.108,P=0.793). the morbidity of AS was 15.79% in MS with HCMV positive group, which is higher than that of MS without HCMV infection group. (χ2= 4.996,P=0.025).By multiple logistic regression analysis , adjusting for sex, age and smoking history and MS parameters, cytomegalovirus infection increased the risk of AS, Odds ratio(OR) was 1.279(95%CI,0.915~1.787,P<0.0001)。.Conclusion: HCMV infection was significantly associated with MS and MS components. cytomegalovirus infection may be an independent risk factor for MS. MS individual with HCMV infection had a higher risk suffering from AS.
