Clinical Research On Activating Blood Circulation And Dissipate Blood Stasis Method In Treating Patients With Traumatic Subarachnoid Hemorrhage

ObjectiveBased on Chinese traditional understanding of pathogenesis of traumatic subarachnoid hemorrhage,this dissertation puts forward that activating blood circulation and dissipate blood stasis method might have a good curative effect in the treatment of tSAH.There are two major purposes of this study.First, this dissertation aims to illustrate the feasibility to apply activating blood circulation and dissipate blood stasis method in the treatment of tSAH.Second, this dissertation proposes a feasible and scientific research plan for the study.It' s likely to give academic and clinical support for further study on life quality improvement of tSAH patients.It also provides some powerful arguments for clinical application of Chinese medicine in tSAH.MethodologyThis dissertation illustrates the feasibility of applying activating blood circulation and dissipate blood stasis method to tSAH treatment and puts forward a research plan through literature review and clinical tests.First, review literatures of Chinese medicine and western medicine.Western literature review illustrates that disposing tSAH accurately is very important in abating cerebral edema and preventing cerebral angio spasm on account of tSAH is the important factor of aggravating secondary brain injury.Chinese literature review shows that activating blood circulation and dissipate blood stasis method can accelerate hemorrhage absorption and abate cerebral edema and prevent cerebral angio spasm effectually.All these are to illustrate the feasibility of applying activating blood circulation and dissipate blood stasis method to tSAH.Second,design a feasible and scientific research plan of clinical tests on the basis of studies of tSAH and activating blood circulation and dissipate blood stasis method.Randomized controlled trial: take 60 cases of matching conditions patients with tSAH through the postulatesed diagnosis,indrawing and excluding standard,through random method divide them into a treatment group and a control group,apiece of 30 cases.The two groups both deal with neurosurgical routine treatment,besides it,the treatment group will be treated taking orally modified tao-hong-si-wu decoction bases on activating blood circulation and dissipate blood stasis method,the controlled group treated with using cerebrocrasts.Through testament there is no significant difference between the two group of patients before treatment in age,sexuality,traumatic condition,GCS,CT Fisher grade etc.ResultsThe literature review and clinical tests have shown the following findings,tSAH engenders by hemorrhage influx of angiorrhexis of bridging vein or cerebral vessels into cavum subarachnoidale cause by craniocerebral injuries.Test of red blood corpuscle in cerebrospinal fluid,CT Fisher grade and clinical objective sign are good assessment tools to observe the curative effect of activating blood circulation and dissipate blood stasis method to tSAH treatment.After 10 days' separate treatments,there are significant difference(p＜0.05 or p＜0.01) between the treatment group and the control group in each aforesaid evaluation criterion,show that activating blood circulation and dissipate blood stasis method can accelerate hemorrhage absorption and abate cerebral edema and prevent cerebral angio spasm effectually because of that trauma devoting stagnated blood is the basic patbogeny and obstruction of collaterals by blood stasis or blood stasis with constipation is the basic pathogenesis of tSAH,conspicuously ameliorate symptoms of patients with tSAH.ConclusionThis dissertation finds that applying activating blood circulation and dissipate blood stasis method in tSAH treatment is feasible.Even if conventional therapy of Western medicine can ameliorate symptoms of patients with tSAH,but therapy of traditional Chinese medicine manifests more preferably.
