Objective To evaluate the cognition degree of knowledge about disease in patients with chronic obstructive pulmonary disease(COPD) in Kunming at the present.Methods A face to face questionnaire survey was conducted among 207 patients, who were diagnosed to suffer from chronic obstructive pulmonary disease in the First Affiliated Hospital of Kunming Medical College,the Second Affiliated Hospital of Kunming Medical College or the Kunming First People's Hospital from March 2008 to June 2008.All the patients agreed to participate in the study.Self-designed questionnaire used in the study consisted of the general condition of patients (including sex,age,nation,height,weight,job,education level,economic status,etc), cognition of COPD about the definition,risk factors,symptom,golden standard,the severity,treatment in acute exacerbation stage or stable stage and the therapy cost. The answers were recorded truthfully and analyzed with SPSS 11.5 to evaluate the cognition degree of knowledge about disease.Results 1.In the cognition degree of definition about disease,there were statistical differences among the 207 patients with different education levels(x~2=17.981,P=0.003),different economic status(x~2= 16.341,P=0.006) and different hospitals(x~2=38.619,P<0.001).2.Among the 207 patients,94.2%had cough,expectoration or dyspnoea,most of patients understood symptoms of disease.3.(1) About the cognition degree of risk factors for COPD, there was no significant differences among the 207 patients with different education levels(x~2=7.436,P=0.684),different economic status(x~2=12.04,P=0.282),but there was statistical differences among the 207 patients in different hospitals(x~2= 20.109,P=0.028).(2) Of the 207 patients,77.3%had ever smoked,average indexes of smoking in these patients was 586±683.There was obviously different among the different sex patients(x~2=143.594,P<0.001).4.The percentage that patients didn't know the golden standard for COPD was 99%.5.About the cognition degree of the severity of COPD,there was no significant differences among the 207 patients with different economic status(x~2=9.325,P=0.316),but there was statistical differences among the 207 patients with different education levels(x~2=17.709,P=0.024), different hospitals(x~2=25.626,P=0.001).6.(1) 73.0%of the 207 patients inhaled oxygen at home,but only 33.8%of the 207 patients thought that the long-term administration of oxygen to patients with COPD was needed.(2) 75.8%of the 207 patients didn't know pulmonary rehabilitation.(3) 34.8%of the 207 patients inhaled aerosols regularly.7.The average hospitalization was once per year at least.The hospitalization expenses of 37.7%of the 207 patients were over 10000 RMB,87.0% expressed that they couldn't afford the therapy cost for COPD.8.Multiple linear regression analysis revealed that the cognition degree of knowledge about disease was affected by many factors.The main factors influencing the cognition were education level and the medical insurance that the patients had or not.Conclusion 1.The 207 patients had a higher cognition on COPD about the symptom,the severity and the therapy cost,but a lower cognition on COPD about the definition,risk factors,golden standard and treatment in stable stage.2.According to different education levels, economic status,and hospitals,the cognition degree of correlated knowledge about disease was different.3.The cognition degree of knowledge about disease was affected by many factors.4.COPD patients had insufficient knowledge about COPD in Kunming,therefore COPD health education should be enhanced and popularized actively.
