Purpose 1.To approach the role of COX-2 in acute pancreatitis associated lung injury. 2.To investigate the influence of Celecoxib,a high special inhibitor of COX-2 activity,on the protective effects in rat of the multi-organ in the severe acute pancreatitis.3.To approach the treatment effect of Radix salviae miltiorrhizae injection united Celecoxib in Severe Acute Pancreatitis Associated Lung Injury.Method SAP rat model was induced by injecting L-arginnie into peritoneal cavity and subcutaneous part.96 Wistar rats was randomly divided into four group.A group,control group(NS group);B group,L-Arg group received intraperitoneal injectiong of 450mg/100g body mass and subcutaneous injection of 200mg/100g body mass with L-arginine at an interval of 2h.NS group received the same volume of 0.9%sodium chloride in the same way.C group Celecoxib was administrated affusedly down alimentary canal from mouth to stomach at 50mg/kg within 1 hour before the inducement of SAP.D group received the same way as C group besides salviae miltiorrhizae injection was injected intravenously after 5mins of the inducement.The rats were killed at(6,12,24hours) after modeling,the following index were tested in four group:the serum amylase and blood oxygen saturation,the ratio of wet/dry with pancreas and lungs:The pathological changes of pancreas,lung were observed with HE dying at the same point,and then gave them pathological scores.The activation level of COX-2 of pancreas and lung was detected by immunohistochemisty with SABC.Results 1.After induceing the model of SPA,the serum amylase,blood oxygen saturation and the wet-dry ratio of pancreas and lung of B group were different from A group,its level was significantly higher than A group(p<0.01),the serum amylase,blood oxygen saturation and the wet-dry ratio of pancreas and lung of group C,although its level is higher than A group,but lower than the B group(p<0.05),The levels of D group were higher than A group,which were lower than B group,and were lower than C group at 12h,24h time point 1(p<0.05).But the blood oxygen saturation has no difference of the group C.2.Each group of pancreatic and lung tissue score:rat pancreas and lung tissue of A group was no obvious lesions under a light microscope.Group B has obvious pancreatic and lung tissue damage changes(p<0.01),C group compared with the B group,obviously alleviate the degrees of pancreas and lung tissue damage in severe acute pancreatitis(p<0.05),D group is lower than C group at 12h,24h point on the pancreas and lung tissue pathology score.3.The expression of COX-2 in each group:There were no COX-2 expression within the pancreas and lung tissue in A group.There were substantial COX-2 positive expression in pancreas and lung tissue of group B,COX-2 expression of group C is lower than B group,but higher than the D group at 12h,24h time point.(p<0.05).4.Correlation analysis:Will be the expression of A.B.C,D groups of the pancreas and the lungs,with the corresponding pathology scores of organ and wet/dry for Spearman correlation analysis,results are as follows:A group:The COX-2 of pancreas was no correlation of wet and dry ratio(r= 0.027,P>0.05),the COX-2 of lungs was no correlation of wet and dry ratio(r= 0.031,P>0.05);also the pancreas pathology scores(r=0.018.P>0.05),and the lungs pathology score(r=0.029.P>0.05)B group:The COX-2 of pancreas was positively related to wet and dry ratio(r =0.568.P<0.05),the COX-2 of lungs was positively correlated to wet and dry ratio(r=0.544.P<0.05):and pancreas pathology scores were positively related to COX-2(r=0.346,P<0.05),and lung pathology scores were positively correlated to COX-2(r=0.389,P<0.05). C group:The COX-2 of pancreas was positively related to wet and dry ratio (r=0.519,P<0.05),the COX-2 of lungs was positively correlated to wet and dry ratio(r=0.530,P<0.05);and pancreas pathology scores were positively related to COX-2(r=0.375,P<0.05),and lung pathology scores were positively correlated to COX-2(r=0.312,P<0.05).D group:The COX-2 of pancreas was positively related to wet and dry ratio (r=0.469,P<0.05),the COX-2 of lungs was positively correlated to wet and dry ratio(r=0.517,P<0.05);and pancreas pathology scores were positively related to COX-2(r=0.297,P<0.05),and lung pathology scores were positively correlated to COX-2(r=0.314,P<0.05).Conclusion The results of this study make it clear that,COX-2 in severe acute pancreatitis associated lung injury plays a key role.The role of COX-2-mediated mechanism may be related to vascular endothelial cell injury and activation. resulting in pulmonary microcirculation and activating the relevant inflammatory response.The treatment of Salvia united Celecoxib has a catalytic role on the inhibition of COX-2,and has a good protective effect in severe acute pancreatitis and pancreatic exocrine organ,its mechanism maybe is related to the inhibition of the expressiong of COX-2.
