| OBJECTIVES:To observe Ankansu' s curative effects and mechanisms on reducing gastrointestinal symptoms caused by tumor chemotherapy, and on bone marrow suppression and to observe the herb' s anti-cancer potency. To evaluate Ankansu' s safety, reliability and practicality and to provide the theoretical basis for the clinical practice.METHODS:1. Observing the frequency of contraction and tension of vitro intestinal smooth muscle to evaluate the effect of Ank-ansu on its changes induced by acetylcholine, histamine, barium chloride, chemotherapeutics and clinically commonly used western and Chinese medicines.2. Observing the facilitating ratio of intestine of mice in each group to evaluate the effect of Ankansu on the changes of fluid emptying in intestine caused by cisplatin and cyclo-phosphamide3. Observing phagocytic rate and phagocytic index of peritoneal macrophages to evaluate the effect of Ankansu on phagocytosis of peritoneal macrophages.4. Observing WBC, RBC, PLT, serum ferritin in peripheral blood to evaluate the effect of Ankansu on the changes of blood cells and serum ferritin caused by cisplatin and cyclo-phosphamide.5. Observing inhibition rate of Ankansu and histopatholo-gical changes in tumor to evaluate anti-cancer efficacy of Ankansu to H22 liver cancer.6. 72 cases of patients diagnosed with tumor, randomly divided into two groups , were given to cisplatinbased conventional chemotherapy treatment and Ankansu was added in the treatment group and metoclopramide in the control group. Gastrointestinal reactions level and KPS, QOL score on quality of life in patients were observed. The efficacy of Ankansu on gastrointestinal side effects induced by cisplatin chemotherapy was evaluated.RESULTS:1. There was no manifest difference on the normal intestinal smooth muscle tension in vitro in low, medium and high Ankansu group(P>0. 05), but the frequency of contraction significantly increased(P<0.05) which was positively correlated with drug dose. Ankansu can relieve significantly the decline of contraction frequency and tension of isolated intestinal smooth muscle induced by cisplatin, cyclophosph-amide, fluorouracil, mitomycin, barium chloride, histamine(P<0.01) .2. The facilitating ratio of intestine of mice in cisplatin group and in cyclophosphamide group degraded significantly (P<0. 01, P<0. 05) and elevated significantly in metoclopramide group and Ankansu group (P <0.05). 3. The phagocytic rate and phagocytic index of peritoneal macrophages in mice increased significantly in low, medium and high Ankansu group compared with the saline control group, cyclophosphamide positive control group(P<0. 01).4. The quantity of white blood cells, platelets of mice in Yixuesheng group and in Ankansu group increased signify-cantly compared with in cyclophosphamide group and cisplatin group (P<0. 05) . RBC(T/L) upgraded obviously in high Ankansu group compared with in control group (P<0. 05) . Ankansu can relieve significantly the decline of serum ferritin induced by cisplatin compared with Yixuesheng (P<0. 05) .5. Unlike CTX group, There is a very significant difference in increased weight of thymus and spleen of mice in medium and high Ankansu group compared with in the saline group(P<0. 01). Ratio of inhibition of high, medium and low Ankansu on H-22 tumor was 65%, 40%, 26%, respectively, and there was a positive correlation between inhibition rate and drug concentration. There was no statistical significance in ratio of inhibition in high Ankansu group compared with in cyclophosphamide group (P>0. 05).6. Compared with the metoclopramide group, there is a significant difference in speed scores of gastrointestinal tract reaction of mice in Ankansu group after a course of chemotherapy(P<0. 01). KPS, QOL score in Ankansu group is sig nificantly different compared with that before treatment (p <0. 01), while there is no statistical significance in the metoclopramide group.CONCLUSIONS:1. Ankansu can stabilize intestine internal environment, increase intestinal peristalsis, promote digestion and absorption of intestine and can regulate the functions of intestinal smooth muscle in vitro.2. Ankansu as well as metoclopramide can relieve significantly the inhibitation of cisplatin and cyclophosph-amide on facilitating ratio of intestine of mice.3. Ankansu can increase significantly phagocytic rate and phagocytic index of peritoneal macrophages in mice and is positively correlated with drug dose.4. Ankansu can relieve significantly the quantity decline of white blood cells, platelets, serum ferritin induced by cisplatin or cyclophosphamide compared with yixuesheng.5. Ankansu can inhibit significantly H-22 tumor via enhancing the immune function.6. Ankansu can improve side effects of digestive tract (e. g. nausea and vomiting ) induced by chemotherapeutic agents such as cisplatin and is more effective than metoclopramide so that it can significantly improve patients' life quality.To sum up, Ankansu can significantly reduce the side effects by the tumor chemotherapy, greatly improve the life quality of patients.The mechanisms are relevant to the herb' s regulation of gastrointestinal function and enhancing the body immunity. The study indicates that Ankansu has a reasonable prescription and is worthy of clinical application. |
