Objective Study on the value of Doppler ultrasound measurement on uterine artery in the late high-risk pregnancy.Methods Fifty-six high-risk pregnant cases and twenty-nine normal pregnant cases whose gestatinal age range from 30 to 40 weeks were examined with Doppler ultrasound. The hemodynamic parameters(S/D,RI,PI) of Ut-A flow waveforms were measured,and the availability of early diastolic notch was recorded at the same time. All the cases would be visited pregnancy prognosis until the pregnancy procedure end. High-risk pregnancy, including fetal growth restriction, abnormal amniotic fluid, pregnancy induced hypertension, gestational diabetes, etc. The hemodynamic parameters and the outcome of pregnancy were compared between high-risk group and normal group, at the same time, the pregnancy outcome was compared between S/D ratio of normal group and abnormal group. The application of Ut-A S/D ratio,early diastolic notch,RI and PI at high-risk pregnancy were evaluated.Results Use of the analysis covariance, between High-risk pregnancy group and normal pregnancy group, there were significant differences of some Ut-A hemodynamic indexes (S/D ratio the incidence of early diastolic notch) (P<0.05),others hemodynamic parameters had not statistical difference (P>0.05).The incidence of low birth weight infants and neonatal asphyxia in the high-risk pregnant group were significantly higher than that in normal pregnant group(P<0.05).The high-risk pregnant group had the worse outcome.The incidence of fetal distress,premature,low birth weight infants and neonatal asphyxia in S/D ratio of abnormal group were significantly higher than that in normal group(P<0.05),but there were no significant differences of abnormal amniotic and fluidgestational age between them.Conclution Ut-A S/D ratio,early diastolic notch are sensitive indexes for predicting high-risk pregnancy outcome, All High-risk pregnant women should be examined Ut-A flow waveforms by CDFI examination in the late pregnancy, by the way, we can understand fetal hemodynamic changes, judge whether intrauterine fetal hypoxia,and help routine antenatal ultrasonography to evaluate high-risk pregnant and predict pregnancy prognosis.
