Research The Law Of TCM Syndrom And Pathogenesis On Lung Cancer And Liver Cancer

Objective:Lung Cancer and Liver Cancer are common malignant tumor in our country residents.Combined therapy is ofen used in clinical currently and TCM play an important role in combined thrapy.Through retrospective document analysis and clinical epidemiology surveys,the research analysis the TCM syndrom of lung cancer and liver cancer in different clinical stage,palmar grasp the distribution of TCM syndrom of lung cancer and liver cancer in clinic, decompose the influence of modern therapeutic tool such as chemotherapy and surgery on TCM syndrome,and study the fundamental factor of TCM syndrome, improve the accuracy of syndrome differentiation-treatment of lung cancer and liver cancer,develop the advantage of TCM therapy and integrated TCM-WM therapy better.The result of our reaserch are as follows:(1)The major style of TCM syndrome of lung cancer:the qi of lung stasis inclusion phlegm and blood stasis;phlegm heat and qi stagnation;spleen asthenia and phlegmatic hygrosis;qi deficiency and phlegm heat;qi-yin both deficiency;yin deficiency and phlegm heat;yang deficiency and phlegmatic hygrosis;yin-yang both deficiency.The proportion are separately 14.4%,4.6%,30.9%,19.6%,13.9%,9.3%,4.6%,2.5%of the whole.(2) The major style of TCM syndrome of liver cancer:spleen asthenia; qi stagnation;dampness-heat;yin deficiency.The proportion are separately 28.6%,9.8%,5.4%,6.3%,24.1%,3.6%,13.4%,7.1%of the whole.On this bottom,further partitioning eight styles of TCM syndrome are as follows:the qi of liver depression and spleen asthenia;spleen asthenia and phlegmatic hygrosis;the qi of liver depression;qi stagnation and blood stasis; hepatochlic hygropyrexia;dampness-heat;liver kidney yin insuf-ficiency,yin deficiency and phlegm heat;The proportion are separately 28.6%,9.8%,5.4%,6.3%,24.1%,3.6%,13.4%,7.1%of the whole. (3) The relationship of the distribution of TCM syndrom and clinical stage: In clinical stageⅠof lung cancer,the qi of lung stasis inclusion phlegm and blood stasis,spleen asthenia and phlegmatic hygrosis are the main syndromes. In clinical stageⅡ,the qi of lung stasis inclusion phlegm and blood stasis, spleen asthenia and phlegmatic hygrosis,qi deficiency and phlegm heat, phlegm heat and qi stagnation are the main syndromes.In clinical stageⅢ, spleen asthenia and phlegmatic hygrosis,qi deficiency and phlegm heat,qi-yin both deficiency;the qi of lung stasis inclusion phlegm and blood stasis are the main syndromes.In clinical stageⅣ,spleen asthenia and phlegmatic hygrosis,qi-yin bothdeficiency,qi deficiency and phlegm heat,yin deficiency and phlegm heat are the main syndromes.In clinical stageⅠof liver cancer, spleen asthenia(the qi of liver depression and spleen asthenia) is the main syndrome.In clinical stageⅡ,spleen asthenia(the qi of liver depression and spleen asthenia),dampness-heat(hepatochlic hygropyrexia) are the main syndromes.In clinical stageⅢ,spleen asthenia(the qi of liver depression and spleen asthenia),dampness-heat(hepatochlic hygropyrexia),yin deficiency(liver kidney yin insufficiency,yin deficiency and phlegm heat) are the main syndromes.(4) The relationship of the styles of TCM syndrom of lung cancer and liver cancer and the modern therapeutic tool:In general,spleen asthenia and phlegmatic hygrosis;qi deficiency and phlegm heat;qi-yin both deficiency are the main syndromes in the first observation of chemotherapy,a week later, spleen asthenia and phlegmatic hygrosis,yang deficiency and phlegmatic hygrosis,qi-yin both deficiency are the main syndromes.Observing a week before and after TACE of liver cance,the qi of liver depression and spleen asthenia,hepatochlic hygropyrexia are the main syndromes before the surgery of TACE,the proportion are separately 28.6%and 21.4%of the whole.After the surgery of TACE,dampness-heat,qi stagnation and blood stasis are the main syndromes,the proportion are separately 32.1%,17.9%of the whole.(5) The study of the fundamental factor of TCM syndrome of lung cancer and liver cancer:The fundamental factor of TCM syndrome of lung cancer and liver cancer are roughly conformity,all of them consisit of qi deficiency, yin deficiency,phlegmatic hygrosis,qi stagnation and blood stasis.But yang deficiency is not seen in the TCM syndrome of live cancer.The percentage of fundamental factor of TCM syndrome of lung cancer are phlegmatic hygrosis (50%)＞qi deficiency(33.5%)=phlegm heat(33.5%)＞yin deficiency(25.8%)＞qi stagnation(19.1%)＞blood stasis(14.4%)＞yang deficiency(7.2%) in turn.The percentage of fundamental factor of TCM syndrome of live cancer are qi stagnation(40.2%)＞qi deficiency(38.4%)=phlegmatic hygrosis(38.4%)＞phlegm heat(34.8%)＞yin deficiency(20.5%)＞blood stasis(6.3%).Qi deficiency and phlegmatic hygrosis are the most common fundamental factor of TCM syndrome in lung cancer,analysis from the clinical symptoms,the qi of spleen asthenia are the mostly common syndrome in the qi deficiency and formed the spleen asthenia and phlegmatic hygrosis.Qi stagnation and qi deficiency are the most common fundamental factor of TCM syndrome in liver cancer,analysis from the clinical symptoms,the qi of spleen asthenia are also the mostly common syndrome in the qi deficiency and formed the qi of liver depression and spleen asthenia.Conclusion:(1) Rensently,most tumor patients who were given the integrated TCM-WM therapy in the middle stage or advanced stage had been done many therapeutic tools,so in the study of the fundamental factor of TCM syndrome of lung cancer and liver cancer,qi deficiency,especially the qi of spleen asthenia is in the dominanace position.Considered it was caused by repeat chemotherapy and injuries the qi of the spleen and stomach.(2)According to the realation between the distribution of TCM syndrom and clinical stage,we discover that the fundmental factor of sthenic syndrome which compose the TCM syndrome are the dominant factor in the early stage,phlegmatic hygrosis,blood stasis and qi stagnation are the main pathological change,in the middle stage,it becomes asthenia-sthenia syndrome,and,yin deficiency, qi deficiency and asthenia-sthenia syndrome are common seen in the last stage.(2) After chemotherapy of lung cancer,the spleen asthenia and phlegmatic hygrosis are also the major TCM syndrome.After the surgery of TACE of liver cancer,dampness-heat,qi stagnation and blood stasis increased than before.(4) To aim directly at the status quo of the integrated TCM-WM therapy of lung cancer and liver cancer,we should use the herbs which can reinforcing qi and strengthening spleen at the first time,and regulation the spleen and stomach after multiple therapiese.Care the spleen and stomach should transfixion the entire therapy all the time.In the last stage of the cancer patients,we should use the herbs which can nourishing yin and reinforcing qi to treatment the syndrome of qi-yin both deficiency,yin deficiency and phlegm heat which caused by Yin-fluid deficiency. The research discussed the distribution of TCM syndrom of lung cancer and liver cancer in different clinical stage,analysis the realationship of TCM syndrome and chincal stage and therapy methods,and analysis the succession law of TCM syndrome,provide the global thread to the syndrome differentiation-treatment in clinical.
