| Objective To observe the protection of Shuxuetong on early diabetic nephropathy and the impact of Shuxuetong on the levels of TGF-β1,TNF-α,IL-6,and MCP-1Methods 60 patients with type 2 diabetes (DM) were selected for this study, including 30 patients with type 2 diabetes without microalbminuria (DM group) and 30 patients with early diabetic nephropathy (DN group).and 20 healthy subjects were used as control group (NC group).The subjects of DN and DM group were administered with Shuxuetong (8ml),QD,14 days as a course of treatment,based on conventional therapy of DM.The levels of serum transforming growth factor-β1(TGF-β1),tumor necrosis factor-α(TNF-α),interleukin-6 (IL-6),urinary monocyte chemoattractant protein-1 (MCP-1),and urinary albmin and urine creatinine for 24 hours (calculated their ratio) (UALB/Cr) were measured before and after treatment as main target;The levels of serum creatinine (Scr),blood urea nitrogen (BUN),glycosylated hemoglobin (HbA1c),β2 microglobuin (β2-MG) of serum and urine,glomerular filtration rate (GFR)and blood fat were detected as secondary target . The content of four cytokines also tested in healthy subjects.Results 1,Compared with that before treatment ,in DN group,the levels of Scr,β2-MG of serum and urine and UALB/Cr significantly decreased (P<0.01),GFR significantly increased (P<0.01). In DM group, Scr (P<0.05),BUN (P<0.01),β2-MG of serum (P<0.01),UALB/Cr (P<0.01) and GFR (P<0.01) had significant difference. Between two groups,β2-MG of urine (P<0.05) and UALB/Cr (P<0.01) had significant difference.2,The levels of TGF-β1,TNF-α,MCP-1,IL-6 in DM and DN group were higher than that in NC group(P<0.01), these cytokines levels in DN group were significantly higher than that in DM group(P<0.01).Compared with that before treatment,the four cytokines levels in DN group significantly decreased;and they in DM group also significantly decreased (P<0.01). Compared between DM and DN group, The levels of TNF-αand MCP-1 significantly decreased(P<0.01). Compared with that in NC group,after treatment,in DN group,the level of TGF-β1 decreaseed (P<0.05) ,the levels of TNF-α,MCP-1 and IL-6 significantly decreased (P<0.01);IL-6 level in DM group also had significant change (P<0.05).3,Compared with that before treatment , in DN group, the levels of TG,TC and LDL-C significantly decreased,the P value was P<0.05,P<0.01 and P<0.01.In DM group, they also had significant change (P<0.01). Compared between the two groups,they had no significant difference.4,Correlation analysis showed (1)the levels of UALB/Cr and diabetic duration,the levels ofβ2-MG of urine,the levels of TGF-β1 and IL-6 are positively correlated.(2)every two levels among TGF-β1,TNF-α,MCP-1 and IL-6 in DM and DN group is positively correlated.Conclusions 1,TGF-β1,TNF-α,MCP-1 and IL-6 may participate in development and progression of DM and DN.2,Shuxuetong can decrease urinary microalbumin of patients with DN,reduce the levels ofβ2-MG of serum and Urine,BUN and Scr,improve the content of GFR.It has the function of renal protection. The action of anti-inflammation may be one of its mechanisms.3,Shuxuetong has the funtion of regulating blood lipids and decrease the content of TC,TG,and LDL-C. |
