Objective:Using Meta analysis of alpha-lipoic acid clinical effect on treatment of diabetic peripheral neuropathy by systematic review. And the analysis of the results guides clinical medication options.Methods:With "diabetic neuropathy","a-lipoic acid","shuxuetong" as the search term, comprehensive search of CNKI, Wanfang full-text database, Chinese biomedical literature database, VIP, CMFD, CDFD and other databases, retrieval time span of January2004-March2014. This included searches of published or unpublished domestic a-lipoic acid treatment of diabetic peripheral neuropathy, randomized controlled intervention application mecobalamin control literature for all clinical trials, exclusion of poor quality, low levels of published literature. Meta-analysis using RevMan5.2software, the results of each study data consolidation and analysis of publication bias.Results:Based on a comprehensive analysis of relevant literature, we include10documents,713patients with diabetic peripheral neuropathy, a-lipoic acid treatment group valid rate significantly higher than the control group of mecobalamin group (OR=0.19,95%CI [0.10,0.28], P<0.01). Median nerve motor nerve conduction velocity improved significantly better than the control group (OR=3.59,95%CI [1.60,5.57], P<0.01). Median nerve sensory nerve conduction velocity improved significantly better than the control group (OR=0.73,95%CI [0.05,1.42], P<0.05). Peroneal nerve motor nerve conduction velocity improved significantly better than the control group (OR=0.92,95%CI [0.71,1.14], P<0.01). Peroneal nerve sensory nerve conduction velocity improved significantly better than the control group (OR=0.79,95%CI [0.21,1.37], P<0.01). In Chinese medicine, statistics famous prescription found that supplementing Qi and nourishing Yin, promoting blood circulation and removing blood stasis of drug use, and more application compatibility insects drugs. And that is the main ingredient of Shuxuetong. Through the five literature analysis,449cases of patients, the results of the joint general tips Shuxuetong lipoic acid group was significantly higher than lipoic acid alone group (OR=0.18,95%CI [0.11,0.24], P<0.01).Conclusion:a-lipoic acid compared mecobalamin can effectively relieve the symptoms of diabetic peripheral neuropathy and improve relevant laboratory parameters. However, due to methodological issues selected literature exists, may affect the authenticity of the results. We still pending further design of rigorous, large sample, multi-center, randomized, controlled clinical trial to validate. Although many studies show that combined traditional Chinese medicine and Western medicine can better improve clinical symptoms, but for further systematic reviews, we need a clear set of TCM syndromes and diagnosis, evaluation criteria.
