The Clinical Research On Diagnosing And Staging Bladder Tumor Preoperation With Real-Time Three-Dimensional Color Doppler Ultrasonography

Objective Using the research of real-time three-dimensional color Doppler ultrasonography on diagnosing and staging bladder tumour,we also tried to investigate the methods of diagnosing and staging bladder tumour before operation.Methods GE Voluson730 3-DUS Doppler,3.5HZ was used.Sixty-eight patients with bladder tumour were examined by two-dimensional ultrasonography and three-dimensional color Doppler ultrasonography.The tumours' number,shape,location,intratumoral vessel and perforating vessel were observed.Phase of tumours were recorded with two-dimensional ultrasonography and three-dimensional color Doppler ultrasonography,both of them were compared with the result of pathology and operation.We also tried to compare the positivity and accuracy of two-dimensional ultrasonography and three-dimensional color Doppler ultrasonography statistically which way was more better on the diagnosis and staging of tumour.Results 98 tumours were found from 68 patients who have had operations and pathological test. It happened more often on the neck,trigular region,left or right lateral wall.It showed there were local thickness of mucus and there were also higher density pillary mass intruding into the lumen of bladder.Besides this,real-time three-dimensional color Doppler ultrasonography is more clear to see the irregularty of tumour besides what we can see in two-dimensional ultrasonography,we can also see cubic morphology and the relation between the base of tumour and the bladder wall from different angles and intratumoral vessel.In our case,the accuracy of two-dimensional ultrasonography and real-time three-dimensional color Doppler ultrasonography are 82.7%and 94.9%respectively.There are significant difference between these two ways(P<0.05).The accuracy of staging before operation is 79%and 90.3%respectively and there are significant difference between them(P<0.05). Two-dimensional color Doppler ultrasonography showed that there were blood dots or blood sticks in the tumour nodes.On real-time three-dimensional color Doppler ultrasonography,the blood signal was found in all tumour nodes.Perforating vessels were found in 62 tumor nodes.The vessel masses were found in 24 tumour nodes.In the rest 12 nodes,there was a little blood signal at the base of them.Conclusion Real-time three-dimensional color Doppler ultrasonography gives a clear stereoscopic view of the lesions,not only shows the configuration,lotion,morphology but also the spatial relationship and extents of lesions from different planes.It's reliable on the diagnosis,staging and showing intratumoral vessel and perforating vessel of bladder tumour.
