Partâ… Clinical symptomatic Characteristics of Lower Functional Digestive DisordersObjective To analyze the clinical symptomatic characteristics of the lower functional digestive disorders (LFDs). Methods Consecutively investigated the patients received the electronic colonoscopies in Wuhan Union hospital endoscope center from August 2005 to May 2006. Diagnosis and analyze the functional digestive disorders in it according to the Romeâ…¡criteria based on the endoscope results. Results Compared with the lower organic digestive disorders (LODs), LFDs is predominant with female patients (P<0.05), the average age is younger than LODs (P<0.05). In the patients above 40 years old, the number of LFDs patients decreased with the increasing of age, however, the number of LODs increased. In the patients above 60 years old, the number of LODs patients is larger than LFDs. Compared with LODs, the incidence rates of abdominal pain, abdominal bloating, abdominal discomfort, diarrhea, constipation and incomplete evacuation are higher in LFDs patients (P<0.05), however, the incidence rates of alternation bowel movements, anus tenesmus, defecation urgency and assistant defecation have no statistic differences in LFDs and LODs (P>0.05). Upper digestive symptoms incidence rates in the LFDs is higher than LODs(P<0.05). The incidence rates of upper abdominal pain, upper abdominal bloating, nausea, appetite lost, early satiety, acid reflux, regurgitation and heartburn are higher in LFDs patients (P<0.05), however, the incidence rates of upper abdominal discomfort and vomit have no statistic differences in LFDs and LODs (P>0.05). Conclusions The clinical symptomatic characteristics of LFDs are different from LODs. These symptom differences are very helpful to the diagnosis and differentiation of the lower digestive diseases. Partâ…¡Clinical Symptomatic Characteristics of Irritable Bowel SyndromeBackground Functional disorders are very common in the outpatients of gastroenterology department, especially, irritable bowel syndrome (IBS) is the most important disease in all these functional disorders. Analyze the symptomatic characteristics of IBS and its three subtypes (C-IBS, D-IBS,A-IBS) can give us good instructions about the diagnosis of it. Aims Analyze the clinical symptomatic characteristics of IBS and its three subtypes. Methods Consecutively investigated the patients received the electronic colonoscopies in Wuhan Union hospital endoscope center from August 2005 to May 2006. Diagnosis and analyze the IBS patients in it according to the Romeâ…¡criteria based on the endoscope results. Results There are 578 IBS patients together, C-IBS subtypes are 250 (43.25%), D-IBS subtypes are 290 (50.17%), A-IBS subtypes are 38 (6.57%). 251 (43.43%) patients are male, 327 (56.57%) patients are female. The number of IBS patients increase with increasing of the age before 40 years old, but the number decrease after 40 years old. The peak age of the onset of IBS is 30 to 40 years old. Compared with D-IBS, C-IBS and A-IBS are predominant with female patients. The incidence rates of abdominal pain, abdominal bloating and abdominal discomfort relieved after defection are different in the three subtypes (P<0.05), but the incidence rates of abdominal discomfort and anus discomfort have no differences (P>0.05). Lower abdominal pain is the most common symptom in all the three subtypes. There are 316 (54.67%) patients with upper digestive symptoms in 578 IBS patients. The incidence rates of nausea, decreased appetite, early satiety, acid reflux and heart burn are different in the three subtypes (P<0.05), but the incidence rates of upper abdominal pain, upper abdominal bloating, upper abdominal discomfort, vomit and regurgitation have no differences (P>0.05). Conclusions Each of the three subtypes of IBS has its feature clinical symptoms. Upper and lower digestive symptoms overlapping in IBS patients is very common. These symptom differences are very helpful to the diagnosis and classify of the disease. Methods Consecutively investigated the patients received the electronic colonoscopies in Wuhan Union Hospital endoscope center from August 2005 to May 2006. Diagnosis the functional digestive disorders in it according to the Romeâ…¡criteria based on the endoscope results, and analyze the gender-related differences of clinical symptomatic characteristics of LFDs. Results (1)Of the 1986 consecutive patients, 1820 actually responsed to the questionnaire, the response rate was 91.64%. There were 952 LFDs in it, 493 were women and 459 were men, women were predominant in LFDs. The average age of women was older than men(P<0.05), the time of course had no difference between them (P>0.05).In all the LFDs patients, both men and women were predominant with the age 35 to 40 years old. In the patients below 40 years old men were predominant while in the patients above 40 years old women were predominant. (2) IBS was the most common disease in LFDs in both women and men. D-IBS was predominant in men, while C-IBS was predominant in women. There were more functional diarrhea patients than functional constipation patients in men, while in women, the later was more than the former. (3) In all the bowel symptoms, the incidence rates of abdominal pain, constipation, anus tenesmus, assistant defecation and improvement of abdominal discomfort after defecation were higher in women than men (P<0.05), however, the incidence rates of diarrhea and defecation urgency were higher in men than women (P<0.05).The incidence rates of abdominal bloating, abdominal discomfort, alternation bowel movements and incomplete evacuation had no statistic differences in men and women (P>0.05). (4) Upper digestive symptoms incidence rates in women (50.30%) were higher than men (43.79%) (P<0.05). The incidence rates of nausea, appetite lost, acid reflux, regurgitation and heartburn were higher in women than men (P<0.05), the incidence rates of upper abdominal pain, upper abdominal bloating, upper abdominal discomfort, vomit and early satiety had no statistic differences in women and men (P>0.05). Conclusions There were differences of age and digestive symptomatic characteristics between men and women with LFDs. To know these differences is very helpful to the diagnosis and differentiation of the lower functional digestive disorders.
