The Effects Of Different Qigong Practice To Sleep Quality And Psychological State Of Type 2 Diabetic Patients With Insomnia

Object:To observe the effects of different Qigong practice to sleep quality and psychological state of type 2 diabetes patients with insomnia.Method:To divide 90 out-patients with type 2 diabetes mellitus and insomnia into three groups,namely Baduanjin Qigong group,Liuzijue Qigong group,and the controlled group.The Baduanjin Qigong practice group do Baduanjun and Relaxation Qigong.The Liuzijue Qigong practice group do Liuzijue and Relaxation Qigong.The first two groups are intervention groups which received routine diabetic treatment combined with the intervention of Qigong practice.And the controlled group only received routine diabetic treatment.All the patients are measured by PSQI,SDS,SAS, HbA1c,FBG and P2hPG in their pretreatment,two months after treatment,four months after treatment.All the statistics are analyzed by software SPSS13.0.Result:1Glucose metabolismAmong all the samples in this two Qigong groups,14 patients(28.6%) have a better level of HbA1cl;20 patients(40.8%)have a good level of HbA1cl;15 patients (30.6%)have a bad level of HbA1cl.Comparison among groups After four months of Qigong practice,the level of FBG, P2hPG and HbA1c show no significant change among groups(P>0.05).To the patients who have bad level of HbA1cl,the HbA1c were improved significantly in the Baduanjin Qigong group compared to the controlled group (P<0.05).There is no significant change in other factors(P>0.05).Differences among groups After four months of Qigong practice,there is no significant difference among groups on FBG,P2hPG and HbA1c(P>0.05). Comparison within group After four months practice,the FBG and P2hPG were significantly improved in the Baduanjin Qigong group(P<0.05).The HbA1 and P2hPG were significantly improved in the Liuzijue Qigong group(P<0.05).The HbA1 were significantly improved in the controlled group(P<0.05).There is no significant change in other aspects(P>0.05).2Depression and anxiety The score of SDS and SAS of the intervention groups are significantly higher than ordinary models in China(P<0.05) before receiving Qigong practice treatment.Among all the samples in this two Qigong groups,compared to the ordinary models in China,37 patients(75.5%) have higher score of SDS;42(85.7%)have higher score of SAS;36 patients(73.5%) have the both.After four months of Qigong practice,the score of the SDS and SAS decreased in the intervention groups,but there is no significant difference between the intervention goups compared with the ordinary models in China(P<0.05)).Comparison among groups After four months of Qigong practice,there is no significant change in the score of SDS and SAS(P>0.05).Differences among groups After four months of Qigong practice,the score of SDS in the Baduanjin Qigong group decreased and the same score in the controlled group increased.There is significant difference between the Baduanjin Qigong group and the controlled group(P<0.05).There is no significant change in other factors (P>0.05)Comparison among groups After four months of Qigong practice,the score of SDS, are significantly better in the Baduanjin Qigong group than in the controlled group (P<0.05).There is no significant change in other factors(P>0.05).3Sleep Quality Among all the samples in this two Qigong groups,48 patients(98.0%) have poor subjective sleep quality,48 patients(98.0%)have different kinds of sleep disturbances;42 patients(85.7%)have daytime dysfuctions;43 patients(87.8%) have long sleep latency.The global sum of PSQI,subjective sleep quality,sleep latency, sleep disturbances,daytime dysfunction of the intervention groups are significantly higher than ordinary models in China(P<0.01) before receiving Qigong practice treatment.After four months of Qigong practice,the global sum of PSQI and sleep latency were improved dramatically which have no significant difference to the ordinary models in China(P>0.05).The habitual sleep efficiency is significantly better than the ordinary models in China(P<0.01).Comparison among groups After four months of Qigong practice,the global sum of PSQI,subjective sleep quality and sleep disturbances are significantly better in the Baduanjin Qigong group than in the controlled group(P<0.05).There is no significant change in other factors(P>0.05).Differences among groups After four months of Qigong practice,the score of subjective sleep quality in the Baduanjin Qigong group and Liuzijue Qigong group decreased and the same score in the controlled group increased.Both Baduanjin Qigong group and Liuzijue Qigong group has significant difference compared with the controlled group(P<0.05).The global sum of PSQI and sleep disturbances are significantly better in the Baduanjin Qigong group than in the controlled group (P<0.05).There is no significant change in other factors(P>0.05)Comparison within group After four months of Qigong practice,the global sum of PSQI,subjective sleep quality,sleep latency,habitual sleep efficiency,sleep disturbances,daytime dysfunction,actual sleep latency and actual sleep duration were improved significantly in the Baduanjin Qigong group(P<0.05).The global sum of PSQI,subjective sleep quality,sleep latency,habitual sleep efficiency,actual sleep latency and actual sleep duration were improved significantly in the Liuzijue Qigong group(P<0.05).There is no significant change in other factors(P>0.05).4Depression and anxiety combined with sleep qualityDifferences among groups To the patients who have both high score of SDS and SAS,the subjective sleep and the score of SDS were improved significantly in the Baduanjin Qigong group compared to the controlled group(P<0.05).The score of subjective sleep decreased in the Baduanjin group and the same score increased in the controlled group.There is no significant change in other factors(P>0.05).5Correlation analysis The difference of the global sum of PSQI has significant positive correlation with the intervention,the difference of the GSI of SCL-90 and the score of SDS and SAS(P<0.05).It has no significant correlation with the difference of the HbA1c,FBG and P2hPG(P>0.05).Conclusion:1.After 4months' of practice,Baduanjin combined with relaxation Qigong may has a effect of decreasing the FBG and P2hPG.To the patients who have bad level of HbA1cl,it may has a effect of decreasing the HbA1cl.Liuzijue comined with relaxation Qigong may has a effect of decreasing the HbA1cl and P2hPG.We consider that long time of Qigong practice may has effect to improve the glucose metabolism.2.The depression and anxiety state of all type 2 diabetic patients with insomnia in this research are relatively poor.The depression and anxiety state are worse than the ordinary people.Qigong practice improves the depression state of type 2 diabetic patients with insomnia at certain level.3.The sleep quality of all type 2 diabetic patients with insomnia in this research is relatively poor,especially in subjective sleep quality,sleep latency,sleep disturbances and daytime dysfunction,etc.Qigong practice improves the abnormal sleep quality of type 2 diabetic patients with insomnia at certain level.4Our research primarily shows that different types of Qigong practice have different effects.As for the sleep quality,Baduanjin combined with relaxation Qigong has better effect in improving different kinds of sleep disturbances,such as abnormal feeling of too cold,too hot,too much hurt and too much nightmare,etc.As for the depression and anxiety,Baduanjin combined with relaxation Qigong has better effect in improving the depression state.Although the result is not enough to get a certain conclusion,due to the limited time,it inspires how to select appropriate Qigong practice and can be a guidance in the further study of Qigong practice.From the all above,we can see that Qigong practice improves the glucose metabolism,depression and sleep quality at certain level.It has a certain clinical value.
