Background:Diffuse axonal injury(DAI) is a kind of traumatic brain injury with very high mortality and disability rate . Now we have been lacked of a sort of convenient and effective method on real monitoring the brain edema had by DAI in clinical. Thereby,the mechanism and the treatment of DAI have been the emphases and difficulty which were discussed and researched in the field of traumatic brain injury all the same. BORN-BE Nonivasive Brain-Edema Monitor(BNBEM) is an instrument which reflecting brain edema passing through the principle of electric field interference,and it can reflect the state of the brain edema in DAI.Objective: To investigate the rule of brain edema in DAI through by the use of BNBEM to the DAI patient; And evaluate the worths of BNBEM instrument in the clinical application; To observe and validate the treatment significance of brain edema by through ventriculus lateralis cerebri derivation(VD).Methods: 26 DAI patients were choosed in this study who comes from Neuro-ICU in Chinese People Armed Police Force Medical College Affiliated Hospital(CPAPFMCAH).All the patients administerd right ventriculus puncture were divided into two group,one contains 13 patients is therapeutic group(TG) and the other is comparing group(CG). All the patients' intracranial pressure(ICP) were monitored within the time in study. The TG was administered sustaining cerebal spinal fluid(CSF) external drainage and non-CSF drainaged in CG,moreover, both two groups were administered the equal quantity and same frequency dehydration medicine, transfusion and brain-protection treatment etc. BORN-BE was used on the both two groups after operation everyday to inspect circumstance of brain edema, observing 7 days continuously.Result: Two groups of patient brain-edema interference coefficient(BEIC) were changing stably while the brain-edema zygomorphous. The interference coefficient(BEIC) of therapeutic group(TG) was lower down than the comparing group with discrepancy having been had statistics meaning.Conclusion: The brain-edema was zygomorphous in DAI patients in basically. The brain-edema had been changing first hightened then depressed following the time ,the peak-hour is being of 3th and 4th days; Drainage CSF can be effective to alleviate the peak value of TBI-brain-edema peak-hour; It can be effective and more sensitive to reflect brain-edema changing condition by use of BORN-BE instrument which have certain application value in clinical. Implement ICP monitoring can duly well and truly display the diversification of ICP after TBI,however, we can't confuse it with brain-edem else. |
