The Value Of CEEG And Xingnaojing In The Patients With Supratentorial Intracerebral Hemorrhage

[Objectives]The aims of this study were to explore the value of CEEG in the evaluation of the pathogenetic condition,short-term curative effect and long-term prognosis of the patients with supratentorial intracerebral hemorrhage(SICH),to investigate the effect of Xingnaojing in the base of conventional therapy on the EEG,short-term curative effect and long-term prognosis of the patients with SICH.[Methods]63 patients with SICH,aged from 40 to 89,were collected from November 2006 to February 2008 in the department of neurology of the first affliated hospital of Soochow University.According to Glasgow Coma Scale(GCS),all the patients were classified into there groups:GCS13~15,GCS9~12 and GCS3~8.The first two groups were then randomly assigned to treat with conventional therapy or Xingnaojing in the base of conventional therapy,the last one was all assigned to treat with Xingnaojing in the base of conventional therapy.All the patients were checked by CEEG in 24 hours of admission and 10 days after admission.Relationship between the degree and changes of EEG and the pathogenetic condition were evaluated by correlation analysis.And make the analysis of the changes of EEG and the pathogenetic condition between the group treated with conventional therapy and the one treated with Xingnaojing in the base of conventional therapy.[Results]1.The first EEG grading was significantly positive correlation(χ2=58.05,P<0.01) with the pathogenetic condition,no correlation(χ2=5.20,P > 0.05) with the short-term curative effect,significantly negative correlation(χ2=35.67,P<0.01) with the long-term prognosis. 2.In GCS 13 to 15 group,the changes of EEG were no correlation(P>0.05) with the short-term curative effect,significantly negative correlation(P<0.01) with the death rate but no correlation(P>0.05) with the rate of favorable prognosis;In GCS 9 to 12 group,the changes of EEG were positive correlation(P<0.05) with the total effective rate but no correlation(P>0.05) with the noticeable effective rate,positive correlation(P<0.05) with the rate of favorable prognosis but no correlation(P>0.05) with the death rate;In GCS 3 to 8 group,the changes of EEG were no correlation(P>0.05) with the short-term curative effect and the long-term prognosis.3.Between the group treated with conventional therapy and the one treated with Xingnaojing in the base of conventional therapy,the noticeable improvement rate and the total improvement rate of EEG both had no statistics significance(P>0.05) in GCS 13 to 15 group while the noticeable improvement rate of EEG had no statistics significance(P>0.05) but the total improvement rate of EEG had statistics significance(P<0.05) in GCS 9 to 12 group;the noticeable effective rate and the total effective rate both had no statistics significance(P>0.05) in GCS 13 to 15 group while the noticeable effective rate had no statistics significance(P>0.05) but the total effective rate had statistics significance(P<0.05) in GCS 9 to 12 group;the rate of favorable prognosis and the death rate had both no statistics significance(P>0.05) in GCS 13 to 15 group and GCS 9 to 12 group. 4.There were 7 patients in GCS 3 to 8 group.Their improvement rate of EEG(14.29%), noticeable effective rate(total effective rate)(14.29%) and rate of favorable prognosis(14.29%) were all low,but their death rate(57.14%) was high.[Conclusions]1.We could straightly evaluate the pathogenetic condition by early CEEG monitoring and EEG grading,which was helpful for clinical diagnosis and therapy of patients with SICH. 2.By CEEG monitoring many times,we analyzed the first EEG grading and the changes of EEG of the patients with SICH.The former was significantly valuable to signify the long-term prognosis but might be helpful to evaluate the short-term curative effect while the latter was helpful to signify the long-term prognosis in GCS 13 to 15 group,valuable to evaluate the short-term curative effect and the long-term prognosis in GCS 9 to 12 group.The first EEG grading united the changes of EEG would evaluate the short-term curative effect and the long-term prognosis of the patients with SICH more accurately and efficiently.3.In the base of conventional therapy,Xingnaojing was valuable to the improvement of EEG and the short-term curative effect but might be helpful to the improvement of the long-term prognosis of the patients with SICH in GCS 9 to 12 group;Xingnaojing might be helpful to the improvement of EEG,the short-term curative effect and the long-term prognosis of the patients with SICH in GCS 13 to 15 group;Xingnaojing might be helpless to the improvement of EEG,the short-term curative effect and the long-term prognosis of the patients with SICH in GCS 3 to 8 group.So in clinical treatment,Xingnaojing was valuable to the patients with SICH,especially in GCS 9 to 12 group.
