Background Mild cognitive impairment (MCI) is a clinical condition that between normal aging and mild dementia. It refers to individuals with cognitive deficits but who do not fulfill a diagnosis of dementia, and daily life functions are not or mildly impairment. It can be divided into two subtypes: amnestic MCI (aMCI) characterized by reduced memory, and non- amnestic MCI (naMCI) in which other cognitive functions rather than memory are mostly impaired. aMCI seems to represent an early stage of AD, while the outcomes of the naMCI subtypes appear more heterogeneous - including vascular dementia, frontotemporal dementia or dementia with Lewy bodies. A considerable number of subjects with MCI develop dementia while a relative few do not .In these years, researches have spurred major therapeutic interest in establishing preclinical prognostic markers for the progression from MCI to dementia. Individuals predisposed to evolve to dementia could benefit of a treatment in an early stage of the disease, delaying their evolution a limited number of years, which could improve their quality of life.Objective To investigate the clinical manifestation and imaging of mild cognitive impairment and Alzheimer disease, for early diagnosis and intervention.Methods Clinical data of thirty MCI and AD cases were retrospectively analyzed.Results Of the twenty-five AD patients, age range of onset were from fifty to seventy-nine years old , and most of the patients had low education level. At early stage, they mainly manifested reduced memory, When developed to AD, their other cognitive functions rather than memory were also impaired , and MMSE scores were less than normal. The MRI often shows cortical atrophy. Of the five MCI patients, they manifested reduced memory only, and MMSE scores were normal.Conclusion the early period of AD manifested reduced memory, when developed to advanced stage, it is hard to recover. So early diagnosis and intervention are very important.
