Clinical Study On The Relationship Between Syndrome-classification And Prognostic Indexes, In Patients With Stable COPD Disease

Objective:To investigate the relationship between syndrome-classification and prognostic indexes,in patients with stable COPD disease.Methods:120 cases with stable COPD disease were differentiated into 3 groups equally according to lung-qi deficiency,lung-qi and spleen-qi deficiency,lung -qi,spleen-qi and kidney-qi deficiency.some detail,such as,the course of disease,weight and height,were recorded.And Quality Of Life(QOL),Medical Research Council dyspnea scale (MRC),6 minutes walk test(6MWT)ware also detected.The differences of above indexes among 3 group ware observed.Results:1.The course of disease in lung-qi,spleen-qi and kidney-qi deficiency group was longer than those in lung-qi deficiency group and in lung-qi and spleen-qi deficiency group significantly.2.Compare to Lung-qi deficiency group,the level of FEV1%,in lung-qi and spleen-qi deficiency group,were decreased.And it was more serious in lung-qi,spleen-qi and kidney-qi deficiency group.In lung-qi,spleen-qi and kidney-qi deficiency group,the level of FVC%is lower than that in lung-qi deficiency group or in lung-qi and spleen-qi deficiency group.The level of FEV1/FVC% in lung-qi deficiency group is higher than that in other two groups.3. In lung-qi and spleen-qi deficiency group,the scores of daily life, depressive symptoms,were higher than those in lung-qi deficiency group, but there were no differences on total quality of life and social activities, anxiety.In lung-qi,spleen-qi and kidney-qi deficiency group,the scores of total quality of life,social activities,were higher than those in lung-qi and spleen-qi deficiency group,but there were no differences on depressive symptoms and anxiety.There were significant differences on all of total quality of life,daily life,social activities,depressive symptoms and anxiety between lung-qi,spleen-qi and kidney-qi deficiency group and lung-qi deficiency group.4.In lung-qi,spleen-qi and kidney-qi deficiency group,the score of BMI(body mass index),was higher in lung-qi deficiency group.The score of BODE index(a prognosis index,measure of body mass index,airflow obstruction,dyspnea,and exercise capacity), dyspnea,and 6MWT(6 minute walk test)went up from lung-qi deficiency group to lung-qi and spleen-qi deficiency group,and to lung-qi,spleen-qi and kidney-qi deficiency group.5.BODE index and the course was positively correlated,the longer the duration,the higher score of BODE index.Conclusions:1.The course of disease in lung-qi,spleen-qi and kidney-qi deficiency group was the longest,followed by lung-qi and lung-qi Spleen-qi Deficiency group.2.In spleen-qi and kidney-qi deficiency group, it had a significantly decline in the pulmonary function test,much more serious obstructive ventilation dysfunction,compare to lung-qi deficiency group,and lung-qi,spleen-qi and kidney-qi deficiency group had even more.3.Compare to Lung-qi deficiency group,lung-Qi Spleen-Qi deficiency patients had no differences on total quality of life,but the daily life abilities was decreased and more serious symptoms of depression,in lung-qi,spleen-qi and kidney-qi deficiency group,quality of life,daily life,social activities reduced,and also more serious symptoms of depression and anxiety.Compare to lung-Qi Spleen-Qi deficiency patients, lung-qi,spleen-qi and kidney-qi deficiency patients had a lower quality of life,and social activities reduced.4.Compare to Lung-qi deficiency group,Lung-qi,spleen-qi and kidney-qi deficiency patients had more serious malnutrition.Among the three group,Lung-qi,spleen-qi and kidney-qi deficiency one had most serious dyspnea,and least social activities,highest probability of re-hospitalization and mortality rate.There is a certain relevance between course of COPD and its prognosis, the longer the duration,the worse the prognosis..
