Objective To observe the analgesic effect and untoward reaction of lappaconitine injection in patient-controlled intravenous analgesia (PCIA) after spinal operation.Methods sixty patients after spinal operation were randomly divided into tow groups with 30 cases each. PCIA in control group received single fentanyl 0.013mg/kg, experimental group received fentanyl 0.0067mg/kg + lappaconitine 40mg or 48mg, the drug of each group were mixed to 100 ml with normal saline for PCIA. Visual analogue scales (VAS) and adverse effects were observed at 6, 24, 48 h postoperatively.Results Pain score in control group and in experimental group were similar. The adverse effects such as vomiting and nausea were fewer in experimental group than that in control group. But other adverse effects such as respiratory depression, urine retention, time of evacuating delay, skin rash were the same.Conclusion PCIA with fentanyl can provide surely postoperative analgesia for spinal operation, PCIA with lappaconitine plus small dose of fentanyl also can provide the same effect and decrease the dose of fentanyl, at the same time, diminish the adverse effects and increase the safety.
