Objective: Biliary factor is the primary factor of the incidence of acute pancreatitis or cholangitis. In recent years, biliary tract disease increased incidence of domestic and foreign scholars of great concern in the incidence of such diseases, the development process, on the cellular and molecular level Trigger mechanism of the disease is not very clear. Now the NF-κB activation and the activation of trypsin are two very important mechanism. This stady was to detain the expression of the content of TNF-αand the NF–κB mRNA in serum (blood) and bile to detection of biliary acute pancreatitis.Methods: We divided our patients into three group: the group of patients with biliary acute pancreatitis( BAP) had 15 cases ; he group of patients with acute obstructive suppurative cholangitis(AOSC) had 10 cases ; he group of patients with Gallstones(G) had 12 cases ;Elisa technique was used to detected. the content of TNF-a in serum and bile, NF -κB mRNA were detected by RT-PCR in blood and bile.Results: We found that the content of TNF-αin serum of the patient with BAP was 135.16±28.02 pg*ml-1, the content of TNF-αin bile of the patient with BAP was 89.40±12.69 pg*ml-1 ; the content of TNF-αin serum of the patient with AOSC was 192.25±26.70 pg*ml-1, the content of TNF-αin bile of the patient with AOSC was 136.18±25.98 pg*ml-1; the content of TNF-αin serum of the patient with Gallstones was 87.66±11.02 pg*ml-1, the content of TNF-αin bile of the patient with Gallstones was 64.22±14.06 pg*ml-1. We also found that the Optical density ratio of NF -κB mRNA in blood of the patient with BAP was 2.11±0.54 the Optical density ratio of NF -κB mRNA in bile of the patient with BAP was 1.05±0.21; the Optical density ratio of NF -κB mRNA in blood of the patient with AOSC was 2.64±0.64, the Optical density ratio of NF -κB mRNA in bile of the patient with AOSC was1.31±0.24; the Optical density ratio of NF -κB mRNA in blood of the patient with Gallstones was 0.94±0.31, the Optical density ratio of NF -κB mRNA in bile of the patient with Gallstones was 0.51±0.26.We also found that the content of TNF-αand the expression of NF -κB mRNA in the patients with BAP was higher than the patients with gallstones(P<0.01), but lower than the patients with AOSC(P<0.01).Conclusion:1.The expression of TNF-αin blood and bile were higher in both AOSC and BAP groups.2. The expression of NF -κB in blood and bile were higher in both AOSC and BAP groups.3.The activation of NF–κB and TNF-αcan promote the formation of inflammatory cytokine ,further increase pancreatic tissue injury and inflammation, play an important role in the condition and development of biliary tract diseases.
