| Chronic congestive heart failure (CHF) is a common and serious clinical syndrome that deadly impairs people' s health. It is not a single disease, but the final stage of all kinds of cardiac disease. Its morbidity and mortality is high. In recent years, along with bio-psycho-social medical model development, the objective of HF treatment has pay attention to improve the quality of life and long-term reasult. This paper consist of two parts including summary and clinical study. The purpose of the experiment is to evaluate the clinical effect and to explore the influence on quality of life and long-term reasult of CHF by using jiaweishenfu keli.The First Part Literature StudyIn the part of summary, it was systematically reviewed that the opinion of Traditional Chinese Medicine (TCM) to CHF, research Progress of etiology and pathology,syndrome differentiation and treatment variation . There is not a identical conclusion about the name. Generally it is include in such categosies as syndrome of dyspnea , palpitation , edema and so on . Opinion of etiological factor and episode mechanism is different. Every doctor has different opinion of etiological factor and episode mechanism. Majority doctors believe the basic pathogenesis was root deficiency and branch excess , heart (yang) qi play the importment part among five zang organs , and others organs also participate the process. The principle of CHF treatment is trengthening healthy qi and eliminating pathogens. Making a comprehensive survey recently effect of studies , characteristic therapy was summarized treatment based on cause of disease,syndrome differentiation acupuncture,injection,combination of TCM and western medicine and so on .It also reviewed the new study of modern medicine result of pathogenesis and treatment of CHF. Now it is considered that the main reason that cause CHF to be exacerbated from adaptation to maladjustment is the decrease inamount and the change of quality (structure and function) of myocardial cells due to many different reasons .On the treatment, it can divided into drug therapy and non-drug therapy .On the drug therapy, it pay more attention to neuroendocrine antagonist and conversing vetricular remodeling. The widespread use of B-blocker,ACEI,ARB,aldosterone antagonist in chronic heart failure prove it. Many experiments have improve the quality of life and long-term result. The therapy match the objective of HF treatment. But in the process of treatment, it also lead some side-effects compared with western medicine. Some of non-drug therapy is too expensive, it can't use widely. Therefore ,we should go on to explore the treatment of CHF.In this part, it dicuss the evaluation standard and examination method of CHF. it was systematically reviewed that the evaluation standards of clinical effect. But the evaluation standard lay particular emphasis on physiology, In recent years , along with bio-psycho-social medical model development. Evaluating quality of life is a new evaluation standard. To evaluate the clinical effect , combination of long-term effects,short-term effects and laboratory testing items, we should establish comprehensive evaluation system .The Second Part Clinical StudyObjective: To observe the efect of Jiaweishenfu granule on the quality of life of CHF( NYHA III group and II - III group) patients and the prognosis , then to prove whether TCM can improve the quality of life and the prognosis of CHF. Through checking symptoms of the traditional of Chinese medical, the Lee CHF score , cardiac functional grading, scored the Minnesota scale, detected aortic ventricle ejection fraction (EF),LVESD,LVEDD,E/A with ultrasonic cardiogram , observed the influence of rehospitalization and mortality, and to observe two course of treatment is better than one course of treatment or not.Methods: 60 patients with chronic congestive heart failure were randomly selected as objects. We took jiaweishenfu granule curing chronic congestive heart failure on the foundation of usual treatment in the treatment group. All patients check symptoms of the traditional of Chinese medical, the Lee CHF score , cardiac functional grading, scored the Minnesota scale, detected aortic ventricle ejection fraction (EF),LVESD,LVEDD,E/A with ultrasonic cardiogram, observed the influence of rehospitalization and mortality , and to observe two course of treatment is better than one course of treatment or not.Results: On symptoms of the TCM, especially to improve these symptoms, such as palpitation,short breath,coldlimbs,edema,lassitude,chest stuffiness,cough and sputum,turbid unctuous urine,green-blue or purple lips, on Lee scores of heart failure,LVEDD and LVESD decreased, while EF increased, to improve quality of Life, these were significant difference between pretreatment and posttreatment, both after the third month and the sixth month (P<0. 05) . The were significantly different between the treatment group and controlled group ,both after the third month and the sixth month(P<0. 05). Rehospitalization and mortality between two groups were not significantly different(P>0.05).Conclusion: Jiaweishenfu keli shows a good effect in improving clinical symptoms and signs, on Lee scores of heart failure decreased,LVEDD and LVESD decreased, while EF increased, to improve quality of Life, and safety on these patients with chronic heart failure. Jiaweishenfu granule can significantly improve the quality of life of CHF patients.
