A Study On Effect Of "Penthorum Chinense Pursh" On Liver Fibrosis Due To Chronic Hepatitis B And HSC

Objective To observe the curative effect of "Penthorum chinense Pursh" granula on liver function and 3 indexes of liver fibrosis in patients with liver fibrosis due to chronic hepatitis B.To observe the effect of "Penthorum chinense Pursh" extractum on proliferation inhibition and secretion of TGF-β1 and Col I at HSC.To approach some mechanism of action of "Penthorum chinense Pursh" to cure liver fibrosis.And to offer some clinical and experimental evidences for the curative effect on liver fibrosis of "Penthorum chinense Pursh"Methods A randomized controlled trial without double-mind was adopted in the clinical study part.200 patients were divided randomly into a treatment group and a control group.100 cases of control group were treated with conventional therapy.While 100 cases of treatment group were treated with "Penthorum chinense Pursh" granula besides conventional therapy.The liver function and 3 indexes of liver fibrosis were observed,and to compare the disparity of observed indexes between two groups with statistics methods.HSC-T6 was adopted as cell model in the empirical study part.Rats were divided into two groups,normal sodium and "Penthorum chinense Pursh" extractum group.Each group were administrated with normal sodium and "Penthorum chinense Pursh" extractum via gastrogavage to make serum.The HSC-T6 viability was observed by AlamarBlue assay,while the toxicity of "Penthorum chinense Pursh" extractum was measured by MTT assay.The expression of Col I and TGF-β1 mRNA was determined by Realtime PCR.The expressions of Col I and TGF-β1 protein was analyzed by Western Blot.Results The liver function(ALT,AST,γ-GT)and 3 indexes of liver fibrosis (HA,LN,CIV)of two groups were both improved.While the treatment group was superior to the control group(P＜0.05),and without obvious adverse effect furtheremore.AlamarBlue assay indicated that differenent concentration of serum with drug "Penthorum chinense Pursh" extractum can inhibit HSC-T6 proliferation.MTT assay indicated that serum with drug "Penthorum chinense Pursh" extractum was no obvious toxicity to HSC-T6.Compared to normal group,serum with drug "Penthorum chinense Pursh" extractum significantly down-regulated the mRNA and protein expression of TGF-β1 and Col I.Conclusion(1)"Penthorum chinense Pursh" should have a role in treating liver fibrosis,for it can effectively improve liver function and indexes of liver fibrosis in patients with liver fibrosis due to chronic hepatitis B and without obvious adverse effect.It is worth to apply widely to patients with liver fibrosis due to chronic hepatitis B.(2)"Penthorum chinense Pursh" can inhibit HSC proliferation and with no obvious toxicity.It significantly decreased TGF-β1 and Col I secretion at HSC.These are some of the scientific proofs of its clinical effect on treating liver fibrosis.(3)It can inhibit HSC proliferation,down-regulat the expression of TGF-β1,inhibit HSC activation,regulate the intracellular and extracellular signal transducfion pathway of HSC,and decrease Col I secretion and expression.With the inhibition of the deposition of ECM in liver,then it cure liver fibrosis. These may be the mechanism of action of "Penthorum chinense Pursh" to cure hepatic fibrosis.
