| Objective①To investigate the distribution of CYP17 and CYP19 polymorphisms in cytochrome P450c(P450) gene in postmenopausal Han women in Anhui as well as its correlation with bone mineral density(BMD) of radius and the level of serum lipid.②Aimed to investigate the relation between C-reactive protein(CRP),serum lipid level and bone mineral density(BMD) in postmenopausal healthy women All will provide the criteria for diagnosis and treatment of osteoporosis,the theory foundation for prevention and cure of osteoporosis in Anhui,and could give the trace for further study of osteoporosis.Methods①CYP17 and CYP19 polymorphism of P450 gene were analyzed by polymerase chain reaction restriction fragment length polymorphism(PCR-RFLP) in 194 healthy postmenopausal Han women in Anhui.BMD at lumbar spine 2-4 and the femoral neck,Ward's triangle and trochanter were measured by dual-energy X-ray absorptionmetry(DEXA) as well.The relationship between BMD and polymorphism of P450 gene were analyzed by variance analysis.②Blood samples of 135 post-menopausal healthy women were collected from this population.Triglyceride(TG) and total cholesterin(TC) were measured by oxidase method,low-density lipoprotein (LDL) was measured by enzyme method,C-reactive protein(CRP) was measured by immunoenhancement turbidimetry method.Results①The distribution of CYP17 and CYP19 genotypes in postmenopausal Han women in Anhui was as follows:TC:48.4%,CC:32%,TT:19.6%;GA:41.1%, AA:37.9%,GG:21%.The distribution of allele frequencies:C(56.2%),T(43.8%); A(58.2%),G(41.8%).Allele frequencies did not deviate from Hardy-weinberg equilibrium(CYP17:χ~2=0.43,P>0.05;CYP19:χ~2=2.132,P>0.05).②BMD at the Ward's triangle was significantly lower in subjects with CYP17 CC genotype than in the combined group with TC genotype(P<0.05),no association with BMD at lumbar spine 2-4 and the femoral neck and trochanter(P>0.05).No association was found with BMD at any site in subjects with CYP19(P>0.05).CYP17 polymorphism was significantly associated with BMD at the ward's after adjusting for age,height,weight, BMI,menarche age,reproductive age,parity and years since menopause(P<0.05).No significant association was found between CYP19 polymorphism and BMD at any site after adjusting these impact factors(P>0.05).③Stepwise multiple regression was performed to analyze the effect of age,years since menopause,height,weight, menarche age,reproductive age,parity on BMD at any site,BMD at any site was significantly inversely correlated with age(P<0.01),weight positively correlated with BMD in hip(P<0.05),BMD at the femoral neck and trochanter inversely correlated with years since menopause(P<0.05),reproductive age was inversely correlated with BMD at lumbar spine 2-4.④No association was found between serum lipid and any genotype of CYP17 and CYP19(P>0.05).⑤The level of CRP was inversely correlated with BMD of lumbar spine 2-4(r=-0.205,p=0.017),no association with BMD of other site.⑥The level of TC was inversely correlated with BMD of femoral neck(r=-0.210, p=0.015 ),the level of LDL was inversely correlated with lumbar BMD (r=-0.225,p=0.009),the level of TG was inversely correlated with femoral neck BMD (r=-0.275,p=0.001 ) and BMD of Ward's triangle(r=-0.174,p=0.043 ).Conclusions①The distribution of CYP17 and genotypes in postmenopausal Han women in Anhui is as follows:TC,CC and TT;GA,AA and GG.The distribution of P450 had racial diversify.②CYP17 polymorphism in P450 gene may have an influence on BMD in postmenopausal women,which suggests that p450gene is a candidate for the genetic determination of BMD.③Grate age,longer years since menopause,earlier reproductive age are the risk factors and weight is a protective factor of osteoporosis in postmenopausal women.④No association is found between P450 gene and serum lipid in postmenopausal women in Anhui.⑤The increased TC, LDL and TG may be involved in the pathogenesis of postmenopausal osteoporosis.⑥Weather or not CRP could be as a tool for screening osteoporosis or osteopenia in postmenopausal women,it can be validated in other large population studies. |
