Objective The obstructive symptoms associated with benign prostatic hyperplasia (BPH) affect the quality of life in the majority of men over 50. In this study, we examined influential factor of obstructive symptoms associated with benign prostatic hyperplasia; at the same time we do another analysis to evaluate the efficacy and safety of the combined therapy with terazosin and amlodipine for obstructive symptoms associated with BPH.Methods The study is a part of another big research which 637 patients were to be selected and 569 patients were completed 4 weeks follow-up visit, we select 272 patients diagnosed with obstructive symptoms associated with BPH for this combination study. Patients were randomly divided into five groups and given amlodipine 5mg, Terazosin 1mg, Terazosin 2mg, Terazosin 1mg plus amlodipine 5mg, Terazosin 2mg plus amlodipine 5mg. once /per day, for 4 weeks. IPSS obstruction score is that the IPSS questionnaire in 1, 3, 5 and 6 together, as the evaluation of obstructive symptoms. Questionnaire, physical examination and urodynamics collect information,these information determined at baseline and again at 4 weeks as efficacy parameters.Results①The degree of obstructive symptoms increase with the growth of the age; with the Sbp increased, intermittent voiding score increased, and with the Dbp increased, IPSS obstruction score, urine-thinning score, voiding force score increased; the increase in physical strength, the severity of the symptoms of obstructive reduced,with the daily seat time increase, IPSS total score, IPSS obstruction score and urine-thinning score increases; logistic regression analysis showed that by the following six factors have a greater impact on obstructive symptoms: age(OR=1.094,95%CI:1.082~1.138),Dbp(OR=1.127,95%CI:1.022~1.244),sit time(OR=1.062,95%CI:1.041~1.083),drinking(OR=1.083,95%CI:1.042~1.127),the age of first sexual(OR=1.324,95%CI:1.046~1.674)and sexual frequency in 30 years old(OR=1.030,95%CI:1.010~1.051).②The reduction of IPSS and the improvement of average urinary flow rates in the combination group were significantly greater than that in the terazosin or amlodipine group (P<0.05); The improvement about the maximal urinary flow rate and the Qol were better in the combination group, but were not significant among the three groups; There were no statistical differences in three group for reported adverse events; The treatment in hypertension and high age group have more good result. Conclusion①Age,state of high Dbp,the age of first sexual, sexual frequency in 30 years old were important factors for obstructive symptoms associated with BPH; Appropriate increase in physical strength and reduce the daily sitting time maybe protective effect of obstructive symptoms; Drinking was possible influential factors for BPH.②The combination of terazosin and amlodipine was shown to be more effective than Terazosin or amlodipine individually in the treatment of obstructive symptoms associated with BPH, combination group was well tolerated: only 13 patients (11.3%) in the combination group reported adverse events while 13 patients (12.7%) in the amlodipine group and 5 patients (7.2%) in the terazosin group reported adverse events.
