Objective: Hypertensive disorders complicating pregnancy is still one of the main complications of obstetrics which endangers the life safety of mother and infant ,but the mechanism is not clear now. After the study of this disease in the recent years, people have got to know that many factors have lead to the result of it and the basic pathological change is generalized spasm of arteriola, leading to metabolic and oxygen deficiency of tissues. Therefore obstructing its generation and development has become the hot spot of researches in the recent years . The researches have proved that the exceptional change of vasoactive factors, especially vasoconstrictor factors and vasodilator factors, has a compact relationship with the generation of pregnancy induced hypertension diseases. NPY is a kind of peptides, which expresses the most in the cardiovascular system of mammalian, and so as NT. The researches have proved that the increase of NPY and the decrease of NT in patients with hypertensive disorders complicating pregnancy, play a key role in the development of hypertensive disorders complicating pregnancy. The purpose of this study is to discuss the expressions of NPY/NT in the placenta tissues of normal pregnancy and preeclampsia and eclampsia patients and its relationship with the pathogenesis of hypertensive disorders complicating pregnancy. It may help us to determine the etiology and to defend and treat the disease.Method: Placental tissue samples were collected from 175 pregnant women who were in the Second Hospital of Jilin University and Obstetrics and Gynecology Hospital in Changchun City (2004--2007).Among them, there are 125 hypertensive disorders complicating pregnancy patients and 50 healthy pregnant women as a control group . Getting full-thickness placenta tissue(the size is about 1.0cm×1.0cm×1.0cm)when the placenta were expulsed . Fix them in 4%neutral formalin paraffin imbedding. Imbed with paraffin .Chip them consecutively in the size 4μm thick . Strept Actividin-Biotin Complex method was used to detect the expression of NPY and NT in the placenta tissue .Result's standard: Observing results made use of double blind procedure .The positive expressions of NPY/NT showed brown particles in cytoplasm or nucleus. Refer to Hao's comprehensive scoring method: observe the distribution, positive strength and positive rate of NPY/NT protein. First according to the degree of stain score zero, one, two and three respectively. Second according to percent of positive cell score zero(65%) respectively. When half of the sum of percent of positive cell and degree of stain is <0.5,response to immunization is negative , 0.5~1 is positive , 1.5~3 is strongly positive.Use software SPSS11.5 to analyze results of Immunohistochemistry and make use ofχ2 test to compare rate among these groups. (P< 0.05 is the significance standard ).Results: 1. NPY and NT are expressed with one accord in the placenta tissues of two groups. NPY was mainly in trophoblast, next in endothelial cell of capillary and also could be observed in stroma. NT was mainly distributed in trophoblast and there is a few positive cell also could be observed in endothelial cell of capillaryand.2. The expression of NPY in patients with mild preeclampsia were significantly increased than those in normal pregnant women(P<0.05). The expression was increased with progress of hypertensive disorders complicating pregnancy . The expression of in patients with severe preeclampsia were significantly increased than those patients with mild preeclampsia (P < 0.05).There were no significant differential between severe preeclanpsia and eclampsia group(P>0.05).3. The expression of NT in patients with severe preeclampsia were significantly reduced than those in normal pregnant women(P<0.05). The expression in patients with severe preeclampsia were significantly reduced than those patients with mild preeclampsia (P <0.05).The expression was decreased with progress of hypertensive disorders complicating pregnancy .There were no significant differential between severe preeclanpsia and eclampsia group(P>0.05). There were no significant differential between mild preeclampsia and normal group(P>0.05). Conclusion:1. The distribution of NPY and NT in normal placenta tissues suggested that they play important roles in placental physiology throughout healthy pregnancy.2. The expression of NPY in patients with preeclampsia and eclampsia were significantly increased than those in normal pregnant women, but the expression of NT were significantly induced. The expression NT/NPY was decreased/ increased with progress of preeclampsia .This revealed that abnormal production of NT/NPY is one of the factors which make the blood vessel disorder in the placenta of patients with hypertensive disorders complicating pregnancy and have intimated relationship with severity of disease. 3. The expression of NPY in patients with preeclampsia and eclampsia were significantly increased, but the expression of NT were significantly induced. It reveals that disturbance accommodation between NPY and NT may be one component element which induces to generation and development of this disease .4. The results suggests that the levels of NPY increased in patients with PIH, and increased with the severity of hypertensive disorders complicating pregnancy, NPY may increase the BP through to reduce the role of the NT. The results indicated that NPY may play a key role in the development of hypertensive disorders complicating pregnancy.
