Background Multiple myeloma(MM) which is described by doctor Willian Macintyre from British completely for the first time in 1850 is a malignant clone proliferative plasma cell tumor which afected 1ife qulity and survival time severely。The disease has a younger onset age and a lower incidence rate in China than Europe and America。In last years,the incidence rate of MM has been ascending which is about 1/100thousand。The statistical data of hospitals in china shows the female-to-male ratio is about 2:3. The overall survival time of MM p- atients has a very large variation which is ranged from a few of months to more than 10 years , which shows that the prognostic factors of MM so complicated。A number of prognostic factors are explored in 1960s-1980s,such as plasma cell labeling index(PCLI),C - Reactive Protein(CRP ),hemoglobin(Hb),blood calcium,creatinine(cr),peripheral blood plasma cells(PBPC s),chromosome 13 deletion[del(13)],etc. And lots of new staging systems were estab lished.But there are still no unified opinions about the optimal prognostic factors and sfaging system of MM . Objective This study was to exp1or the importance of hemoglobin in effecting on the prognostis of MM by comparing hemoglobin level with other prognostic factors ,and also to exp1or clinical application value in clinical stage of MM ,search for the best clinical prognosis factor of MM . Materials and methods Eighty three previously untreated MM patients which had complete clinical and follow-up records were included in the curent study. All patients -including 52 newly diagnosed patients and 31 newly diagnosed patients by other hospital were seen between February 2000 and August 2007 from the hospital of An Hui province.We gathered related clinical and laboratory parameters when the patients first diagnosed. These MM patients were classified into International staging system (ISS),M protin and hemoglobin levels。In order to com p are their suvival times,and analysis the changes of hemoglobin level and compare hemoglobin level with other prognostic factors。We use Kap1an Meier methods to calculate the accumulated survival rates;Log - Rank analysis were conducted for univariate analysis and group survival rates com parison in above staging system each.Multivariate analysis were caIculated by Cox regression models。Results This study showed all the patients had anemia, hemoglobin levels were negatively correlated with plasm a cell labeling index,C - Reactive Protein,peripheral blood plasma cells ,and positively correlated with serum albumin,there are close relations between hemoglobin levels and the stage of MM.The hemoglobin levels of treated patients was improved obviously, hemoglobin less than 85g/L afected survival times of MM patients .Conclusion As the important prognosis factor of MM, the hemoglobin is indispensable in clinical stages of MM。the changes of the hemoglobin concentration may be used to instruct clinical medication for MM patients...
