Clinical Application Of Ileus Tube In Instestinal Obstruction Treatment

Intestinal obstruction,especially acute ileus is one of very common acute abdominal diseases, its clinical manifestation includes stop defaecation and exhaust air,abdominal pain,vomiting. Surgical operation has been considerd to be the most effective treatment for long time,but a problem that Obstruction will occur again suffer from operation make surgeon hard.Recently medical science and biomaterials artistical have acquired great progress,they promote ileus tube developed and gradually be used to clinical treatment,many inpatients avoid operations from ileus tube.It reduce the incidence rates of operation and improve patients quality of life. This new clinical treat mode already widespread in Japen. There has few reports about clinical application of ileus tube in our country. This article has a retrospective analysis to 30 patients with intestnal obstruction,they all have used nasogastric tube but have few effective,then they all used ilues tube to continue treament, to research the meanings of ilues tube clinical application.Those 30 patients with intestnal obstruction 14 cases were male,16 cases were female,the ages were among 28-78Y,average was 54Y, 26 cases have operation history in abdomen. First,they with nasogastric tube ,unite general expectant treatment ,the average time is 7d(3-10d),the clinical symptoms have few change . With the help of fiber gastroscope ,the ilues tube been put through nose,stomach,duodenum and Treitz ligament,follow the move of ilues tube,to decompress the whole expanded obstruction. Take a X-ray on abdomen per 6 hours. Abdominal distension symptom ,gastrointestinal decompression amounts ,abdominal plain film and the disappearing time of liquid and gas were observed and compared.When ilues tube were placing at average time 2d(1-4d),the pain and distension of the abdominal in patients got significant release.The ilues tube had a significant effect on draining digestive juice (1720±840)ml in first 24h versus nasogastric tube 640±100ml/d,P<0.05.The ilues tube reduced patients'abdominal circumference more markly(14.8±5.0)cm in first 24h,at same time the nasogastric tube were 5.3±3.2cm. P<0.05.The average disappearing time of liquid and gas in the small bowel was 6d.At the end of treatment,25 cases were healing,recovery rate was 83.3%,3 cases got release ,2 cases turn to operate. 24 cases got healing in 26 cases that have operation history in abdomen,the recovery rate was 92.3%.Conclusion:Clinical application of ileus tube in intestinal obstruction can more effectively drain digestive juice, relieve intestinal dilatation, it can be use to confirm the region and origin of obstruction ,reduce the rates of operation,promote achievement ratio of surgery, shorten length of stay in hospital.The ilues tube has a notably treatment effect to the patients with postoperative intestinal obstruction.
