| Cerebral infarction threatens humanbeing's health severely and its mutilation rate and fatality rate are very high in cerebral arterial diseases. How to use different means to evaluate Cerebral infarction's prognosis and offer rehabilitation plan was coucial to the nerology clinician. Recently, with the rapid development of medical instrument, especially, in the field of magnetic resonance function imaging which is very soon. Of which diffusion tensor imaging (DTI) gradually develope rapidly in medical imaging for Cerebral infarction's diagnose was opened broad space.53 cases of cerebral infarction patients with fractional anisotropy(FA)and Diffusion tensor tracking(DTT) appearance in diffusion tensor imaging(DTI) and reference relevant literature make were analyzed retrospectively. With DTI's clinical application value in cerebral infarction diagnose, It could offer a new imageological diagnosis method in clinical cerebral infarction's course monitoring, curative effect and judgement turnover etc.Methods: The study collect 53 cases of different time and different position cerebral infarction patients, who were performed by SIEMENS 1.5T Avanto MR system. T1WI,T2WI,Dark-fluid and DWI were scanned regularly: the slice thickness was 6mm, the slice interval was 1.2mm, the field of view was 230mm, T1WI: TR/TE=400/7.8ms,T2WI:TR/TE=3250/99ms,Dark-fluid:TR/TE=9000 /109ms,TI=2500ms. Diffusion tensor imaging (DTI) used echo planar imaging, all cases were collected through 12 directions diffusion weighted sensitive gradient, b=1000s/mm2, scanning deck parallelmmonMRI,layerthickness:2.8mm,TR/TE=4500/80ms,interval:0.36mm, FOV=230×230, stimulated time: 5, acquisition time: 4minute 19second. All the original data were collected and treated by special DTI tools, which could give FA (fractional anisotropy) map. The change of FA value was analyzed. DTT was performed and bilateral corticospinal tract (CST) was reconstructed by seed method.Results: In common health adult control group, DTI showed that different regional cerebral white matter fiber bundle anisotropism is not different, so it can be seen from FA (fractional anisotropy) value in splenium of corp-us callosum >that in genu of corpus callosum >that in posterior limb of internal capsule > that in anterior limb of internal capsule>that in external capsule. In normal control two side correspond position FA (fractional anisotropy) value average had not obviously difference(P>0.05), two side cerebral white fiber bundle anisotropism roughly was symmetric distribution. FA (fractional anisotropy) value of Cerebral infarction focal significant decreased uninjured side in different time and different position in cerebral infarction group, differences between two side fiber had significance obviously (P<0.01). Statistics analysis by DTT class, example number percentage comparison analysis had statistical significance in cerebral infarction group. In 10 cases cerebral infarction patients, trouble side fibrae corticospinales were compressed, distorted, rarefacted and interrupted and clinical muscle strength appearance did not decrease, post-treatment muscle strength recovery or did not recovery. The relationship to be involved in condition between fibrae corticospinales and muscle strength have dependability (P<0.05).The results showed that:1,Diffusion tensor tracking(DTT) manifest Cerebral infarction focus area prior to common magnetic resonance imaging. In the different locus infarct side FA (fractional anisotropy) value all degrades obviously (P<0.05).2,After brain tissue, FA (fractional anisotropy) value following Infarct time generate regular change: in super-acute stage, trouble side FA (fractional anisotropy) to compare opposite side may step up or degrade lightly. Then FA (fractional anisotropy) value (subacute staged and chronic phase) degrades, then chronic phase continue to degrade.3,Through the research of Diffusion tensor tracking(DTT) class to appraise different time and different position Cerebral infarction patients'prognosis and functional rehabilitation had better clinic significance and application value.4,Diffusion tensor tracking(DTT) show Infarct patient corticospinal tract(CST) degree of injury with muscle strength between of relationship, for clinician offer all the better favourable imageology diagnose evidence. |
