The Researchs On The Protocol For Sedation And Analgesia In Treatment Of Uterine Fibroids By HIFU

Objective:1. To evaluate the safety and feasibility of stepwise protocol for sedation and analgesia (ABCDE protocol, developed by Malone et al.) in treatment of uterine fibroids by HIFU.2. To evaluate the safety and effectiveness of improved stepwise protocol for sedation and analgesia that was developed on the characteristics of HIFU .Methods:Eighty-two Patients with uterine fibroids whose ASAâ… -â…¡were divided randomly into control group (n=20,the drugs were administered based on the reaction of patients to the treatment of HIFU),A group (n=31, the drugs were administered in a regular interval:40min/40min/50min /60 min) and B group (n=31, the drugs were administered in a regular interval:40min/40min/60min/60min).The drugs were administered according to the stepwise protocol for sedation and analgesia (ABCDE protocol ) for the patients of control groups . The drugs were administered for the patients of A group and B group according to the following protocol based on the ABCDE protocol : the first administration: A+B ,the second administration: C+D, the following administration: doses of 25% of the first effective loading dose every time.Results:1.There were no statistical significance for MPA and spo2 and heart rate between before and during treatment for three groups(p>0.05).2.there were statistical significance for respiratory rate between before and during treatment for three groups(p<0.05).3.The sedation scores of each moment were less than 4 class for the three groups. Compare with B group and control group ,the sedation scores of A group were considered to be highest(p<0.05).4.There were no statistical significance for pain scores between A group and B group(p>0.05),the pain scores of A group and B group were less than that of control group(p<0.05).Conclusion:1.The stepwise protocol for sedation and analgesia(ABCDE protocol, developed by Malone et al.) is safe and feasible in treatment of uterine fibroids by HIFU, but the effective of analgesia still need to be improved.2. The improved stepwise protocol that developed on the characteristics of HIFU is more suitable to the treatment of uterine fibroids by HIFU .The effective of sedation and analgesia of A group protocol is the best in three groups.3. The protocol for sedation and analgesia in treatment of uterine fibroids by HIFU can provide the suitable sedation for patients, which facilitate for medical staff to find the harmful stimulation immediately in order to avoid the complications of nerve injury and skin burn.
