Objective : The conception of"social vulnerable group"has become in 2002. The meaning of"social vulnerable population"is those who can not maintain minimum life level by themselves. It includes the poor population, the migrant workers, the laid-off workers, the handicapped, the elderly, the women and the children in the society. This population shows the vulnerability and marginalization in many aspects such as quality of life, income, political affairs, social insurance and health condition and so on, even more badly in rural areas of China. Based on present study, the level of this population is completely lower on health condition, medical service assessment, medical care, social welfare, health insurance and payments than other population. Among vulnerability population, to promote their health level and establish effective health strategies should be very important for rural medical care and disease control.The objectives of present study are to identify the difficulties and barriers in health service of rural vulnerable population, and to provide evidence and suggestions for the interrelated department of medical and health organization. Methods: The questionnaire was designed by study team based on the international standards for evaluation of life quality according to different aim, object and population for investigation.. The data has been Established Microsoft Excel database and inputted. Excel database was transformed in to SPSS database, and analyzed by SPSS13.3. The main statistic method is X2 test and some relative analysis.Results1 The health assessment of vulnerable group in rural areas of Hebei province.The prevalence rate of this group was 91.1%, and the rate of infectious disease, chronic disease, mental disease and trauma were higher than popular people. The result shows that 22.9% people chose no treatment, and about 61.6% forgave treatment and rehabilitation from hospital. The main reason was they can't afford to the medical expenditure. The awareness rate of infectious disease was 31.9%; the parturition rate at home was 48.3%; the gynecological screen rate was 30.1%; the rate of vaccination of children was 26.2% and about 16.1% of them never inoculate. The index of nutriology were lower than the standard obviously.2 The factors impacted the health level and life quality of vulnerable population in Hebei province.Medical expenditure is the important impacted factor for low income family. 88.1% people paid without any support. The rate which people has been involved in cooperating the medical treatment system for rural people is 30.6 % ; the prevalence rate of more income group is higher than the less, and it getting become a vicious circle. Another factor is the age related: the income of over 70-year old is lower than less 30-year obviously, but the prevalence rate and the burden from disease are opposite.Lacking of the medical and social ensurance is also a impacted factor .And then,the lower education made them accepting the health knowleage actively more harder;The distribution of the medical resources don't make them getting the basic health service.3 The policies and strategy on medical ensurance and health service of vulnerable population in rural areas.Promting the economical level and establishing the medical ensurance of this population are the necessary measures to improve their health condition. Establishing the rural health service systerm matching the economical and social development and do some salvation for them also are the effective measures. Enhancing health education can improve their health condition; and the other measures can change the situation are establishing the medical ensurance, retirement systerm, salary systerm and so on.Conclusions: 1 The health index and economical situation of this population in Hebei province are lower than the other population obviously. Because of the heavy burden from disease they chose giving up medical service mostly to get in a vicious circle. 2 There are many reasons for them to be vulnerable population: age and gender, low income and higher expenditure especially in medical care, lack of medical, social and salary insurance systerm.3 The government and interreleted department are establishing policies on medical and health service for this population in order to improve their situation.
