| Background and Objective: IVF-ET treatment has become an important means ofinfertility. Although the clinical pregnancy rate continued to improve, the technologyhas been troubled by high rate of abortion and premature delivery. Reported in theliterature, assisted reproductive technology abortion rate is as high as 25%in the earlypregnancy, but the abortion rate in natural pregnancy is 10%to 20%. Visibility IVFabortion rate is significantly higher than that of spontaneous abortion. Its etiology isrelated with the patients'age, genetic, endocrine, infection, immunization, anatomyand other relevant factors, but a considerable number of patients'reasons for abortionis not clear. So this research is to investigate the decidua' role in the unexplainedabortion patients after IVF-ET programme and study the expression level of maternaldecidual immune-related gene(indoleamine 2,3-dioxygenase, complement factor H,decay accelerating factor), interleukin-8 and phospholipase A2ⅡA, in order to findpossible reasons for unexplained abortion after IVF-ET and provide a theoretical basisfor clinical diagnosis and treatment.Materials and methods: Collect 7 women's decidua as normal control group,whorequired abortion without abortion indication due to unwanted pregnancies. Collect 31women's decidua as study Group,who suffered unexplained abortion after IVF-ET.To comparative study their indoleamine 2,3-dioxygenase, complement factor H,decayaccelerating factor,interleukin-8 and phospholipase A2ⅡA expression. Using RNAblot hybridization techniques to study, and the data was analyzed using statisticalsoftware spss13.0.Results: The relative expression level of immune-related gene CFH and DAF in thecontrol group respectively is 0.68±0.31,0.63±0.35, while 0.60±0.30,0.51±0.31 in thestudy group, there is no significant statistical difference in the two groups (P>0.05).The relative expression level of immune-related gene IDO in the control group is1.39±0.49, in the study group it is 0.78±0.24.Decidual IDO expression in theunexplained abortion patients after IVF-ET is significantly decreased than normal pregnancy women (P<0.05), 100%(31/31) patients'IDO expression in thestudy group is below the normal mean. The relative expression level of interleukin-8in the control group is 0.69±0.29, in the study group it is 0.61±0.17, there is nosignificant statistical difference in the two groups (P>0.05). The relative expressionlevel of PLA2ⅡA in the control group is 1.99±0.32,while 1.28±0.51 in the studygroup, decidual PLA2ⅡA expression in the unexplained abortion patients afterIVF-ET is significantly decreased than normal pregnancy women (P<0.05),93.55%(29/31) patients'PLA2ⅡA expression in the study group is below the normalmean.Conclusion: 1. Decidual IDO expression in the unexplained abortion patients afterIVF-ET is significantly decreased than normal pregnancy women (P<0.05), and allpatients'DO expression in the study group is below the normal mean, theimmune-related gene IDO's decreased expression may be an important reason forunexplained abortion after IVF-ET. 2. Decidual PLA2ⅡA expression in theunexplained abortion patients after IVF-ET is significantly decreased than normalpregnancy women (P<0.05), and 93.55%(29/31) patients'PLA2ⅡA expression in thestudy group is below the normal mean, PLA2ⅡA may play an important role inunexplained abortion after IVF-ET.But it's specific control mechanisms and fuction inall aspects of pregnancy need further research. 3. This research provides new basis forclinical diagnosis, diagnosis method, and possible treatment program. 4.If thedecidual IDO expression decreased in the unexplained abortion patients after IVF-ET,we may try small dose of adrenocorticotropic hormone or percutaneous injectingHusband's lymphocytes. Whether this treatment is effective for these patients, furtherclinical observation is needed. 5. If the decidual PLA2ⅡA expression decreased inthe unexplained abortion patients after IVF-ET, by using estrogen drugs by mouth,vaginal or cervical or taking antimicrobial drugs orally, perhaps could avoid therecurrence of abortion, but further clinical study is needed to confirmed. |
