Objective To study the clinical value of three-dimensional CTangiography(3D-CTA) in the diagnosis and treatment of intracranialaneurysms.Methods 33 patients with suspected to the ruptured aneurysms whodisplayed subarachnoid hemorrhage (SAH) on CT imaging were allexamined with 3D-CTA. We operated directly on them who be found havinganeurysms, the others who not be found having aneurysms were examinedwith 3D-CTA or DSA again. Then we analyzed the cerebral vascular showedby CTA and compared with DSA and operation.Results In all 33 cases, a total of 35 aneurysms were detected in 31cases,two patients each had two aneurysms and one had three aneurysms. In all 35aneurysms, 16 on the anterior communicating artery, 12 on the posteriorcommunicating artery, 3 on the middle cerebral artery, 2 on the internalcarotid artery and 2 on the anterior choroidal artery. Maximal diameter ofaneurysms body was 20mm and minimal diameter was 1 mm. In the 3 caseswho not be found having aneurysm in the first 3D-CTA, one was found inthe second 3D-CTA, while two wasn't in the following DSA. Except 1 case,the others were processed on microsurgical treatment. 3D-CTA results weresame as the surgery. The accurate rate of 3D-CTA in the diagnosis ofintracranial aneurysms was 97% (34/35), the fake negative rate was 3%(1/35), the negative likelihood with DSA was 100% (2/2).Conclusion 3D-CTA can diagnose intracranial aneurysms accurately anddisplay the location, number, size, shape, body and neck of aneurysmsdistinctly, as well as the relationship to adjacent arteries and other anatomicalstructures. According to that, we may operate on the patients directly.3D-CTA should be regard as the first choice for diagnosing intracranialaneurysms, and hopeful to replace DSA, at least partly.
