Clinical Analysis Of The Non-surgical Therapy Of Ocular Chemical Burns

With the development of chemical industry,ocular chemical burns increases gradually.Ocular chemical burns is a kind of ocular corrupted injure which is caused mainly by acid or alkali substance ,is ten percent of ocular trauma ,is five to twenty percent of industrial danger.Acid substance can coagulate protein .Coagulated protein can block the penetration of acid substance ,injure tissue mildly.Alkali substance can dissolve protein and fat,penetrate interior of eye quickly,induce cellular decomposition and necrosis .So the consequence of alkali burns is more serious.But high concentration of acid can induce the same destructive effect as alkali to the body and eye,have intense dehydration effect,result in dehydration and necrosis of cell.The degree of ocular chemical burns is related to category,concentration,dosage,mode of action,contact period,area,temperature,pressure,condition of chemical substance.According to the Roper-Hall's standard graduation of ocular chemical burns:Gradeâ… ,â…¡injures mildly,prognosis is good; Gradeâ…¢,â…£injures heavily,prognosis is poor.Our study is to retrospectively analyse curative effect on different methods of 32 cases (48 eyes) of ocular chemical burns between 2001 and 2007 in our hospital,so as to summarize curative effect of the various non-surgical operation treatments of ocular chemical burns,to improve cure rate .The average age of the patients was 33.2 years old.Simple eye and binocular 16 examples respectively,9 in right eye,7 in left eye.25 cases were workers ,23 cases injured in workplace. Ocular chemical burns usually took place in chemical plant,building site,laboratory,medical therapy unit.Most were caused by work wound .Most lack knowledge of speciality and safeguard ,so injured easily.Ocular burns effects appearance ,reduces the quality of life .It is necessary to improve labour-safeguard appliances and protect the child from danger. Acid burns 16 examples ,alkali burns 14 examples ,others 2 examples.Oil of vitriol was the most primary injurants of acid burns ,lime was the most primary injurants of alkali burns. Oil of vitriol and lime contact ocular surface ,release heat ,increase damage to tissue. The therapy of ocular chemical burns is complicated.Early treatment is first aid,to prevent the spread of necrotic process and infection,to recover the nutrition of injured tissue ,to reduce the ocurrance of complication .Emergency treatment is crucial to prognosis.In this study,30 patients received emergency treatment,whose prognosis was good.It is important to propagandize general knowledge of labour safeguard,ameliorate labour-protect appliances and popularize first-aid treatment of ocular chemical burns under emergent condition. The application of many kinds of medicine together in proper time and proper dosage can shorten corneal repair time and promote corneal healing.The refractory case should be treated with surgical operation.Late treatment is to treat complication and improve the condition of ocular surface.Corneal repair of ocular chemical burns includes epithelial and stromatic repair.In severe chemical burns ,most of limbal stem cells lose,peripheral conjunctival epithelium finishes corneal epithelial repair;corneal stromatic repair decreases.Severe chemical burns induces corneal ulcer and perforation easily.The corneal repair rate in acid burns(96%) was higher than that in alkali burns(91%).Severe chemical burns should be treated with surgical operation actively.Non-surgical operation treatment in early time is necessary to improve the operational surrounding.According to this case ,we can conclude : The average age of the patients was 33.2 years old. Ocular chemical burns was usually caused by work wound . Acid and alkali were primary injurants of ocular chemical burns.Emergency treatment is important to ocular chemical burns.The non-surgical operation treatment is an effective method of curing ocular chemical burns.The curative effect of non-surgical operation treatment is conspicuous to Greadâ… ,â…¡ocular burns.
