| Consultation-liaison psychiatry: also known as liaison psychia try or comprehensive psychiatry is an important branch of Clinical Psychiatry, refer to psychiatric medical care, teaching and research in the general hospital. It focuses on the study of psychological and social factors, the relationship between mental disorders and physical disease, and from the psychological, social and biological aspects of medicine and other branches of science multidimensional diagnoses and treats patients. In our study, CLP is also known as the mental health in the general hospital, a general hospital psychiatry areas.CLP has a history of more than 70 years, created in 1930 before and after. G. Henry and H. F. Dunbar, two students of Adolf Meyer who is the father of American Psychological biology, carried out the general hospital clinical psychiatric study.in the Faculty of Medicine at Cornell University Medical College and Columbia University in New York Respectively. Billings first proposed "Liaison Psychiatry" in 1939. CLP is the natural product of in-patient psychiatry in general hospitals.A series of consultation-liaison psychiatry technolo gy built on the bases of the diagnosis and treatment of neuro-psyc hiatic illness in medical and surgical patients.The task of CLP is to conduct psychiatric medical, teaching and research in the general hospital by psychiatrist, to focuse on the study of the mental health,social factors and the relationship between physical illness and mental disorders of patients in general hospitals, and diagnose and treat patients multidimensionally according to psychological, social and biological factors Specific contents is to provide non-specific psychiatric clinical physician with liaison or consultation services, to raise their ability to identify symptoms ofpsychiatric patients,to carry out psychiatric education and psycho-social education to medical staff, to strengthen the the study of etiology, prognosis ,treatment of disease, a variety of physical illness or psychological trauma and mental illness complications. to apply psychological, sociological, medicinal, biomedical and other treatment technology,and to synergize other sections physicians handle patients, as well as assess the rehabilit ation of patients comprehensively. These are the development direction of CLP, the needs of bio-psycho-social model development.Compared with the United States and European countries, there is a greater gap in our country's consultation-liaison psychiatry work in organizations, the scale of consultation, liaison pattern education and training and scientific research. Mainly manifested in the following: the lack of consultation-liaison psychiatry organizations; low Consultation rates; only consultation without liaison; the low recognition rate on mental disorders by non-psychiatrist.In recent years, with the rapid development of social, closely related to the development of social, the occurrence and solution of psychologic obstacles, modern means of treatment of mental illness, physical illness and mental disorder patients. mental health services and actual demandand so on, more and more attracted to the community's concerns. As an important branch of psychiatry. consultation-liaison psychiatry develop gradually with the appearance of the general hospital mental health services.Its importance has been attached.Currently consultation-liaison psychiatry in the general hospital is mainly based on consultation.clinical departments found psychiatric symptoms in patients and invite psychiatrists for consultation. Psychiatrists give corresponding diagnosis and treatment. To explore further consultation-liaisonpsychiatry status in general hospital, 90 patients from the consultation between 2005 and 2006 were analyzed in this paper to understand the consultation-liaison psychiatry in general hospital status. The results showed: 1, The refered patient's age is olde, the mean age is 51.46 years, total consultation rates is 1.21%, lower than abroad. 2, The primary disease of the refered patient distribute a wide range. Internal medicine was the service that accounted for the majority of psychiatric consultations.neurology (17 cases)account for 18.9%. The most common psychiatric diagnoses for consultation patients were organic mental disorder (66.7%) and neurosis or neurosis-like reaction (12.2%). The most common recommended interventions is antipsychotics with antianxiety drug, accounting for 38.9%.3, Mental health of diagnosis patients is poorer; 4, there are differences between psychiatric outpatients and inpatients in age, disease diagno sisane drug use.Conclusion: The consultation rates of the general hospital is still relatively low. Organic mental disorders and neurological disorders is the main targets of psychiatric consultation and service in the general hospital. The refered patient is in poor mental health. After psychiatric consultation, anxiety is be improved. However, the sense of happiness in the course of treatment without significant changes. There are differences between psychiatric outpatients and inpatients. All these ask general hospital psychiatric doctors should have wide clinical knowledge and skills.To improve mental health services in general hospital inpatients, it is required to strengthen clinical psychiatry and clinical medicine of bio-psycho-social model concept, vigorously carry out face-psychiatrist mental health knowledge training, conduct extensively the public mental health knowledge Dawa. so as to improve the common mental disorders in the earlyidentification and effective treatment and referral rates.Psychiatrists need to strengthen the general hospital doctors and other sections of cooperation in psychiatry at all levels into clinical medicine, meanwhile, strengthen the patients, medical staff and community education in psychiatry clinical practice.It even need the knowledge and clinical skills in clinical practice, including psychiatry,psychology and clinical medicine.Consultation-liaison psychiatry have important value.in the application in the general hospital, and promote the development of hospital and non-psychiatric doctors understand psychiatric,all hospitals raise the ability of treatment of patients. |
