The Application Of Lateral Masses In Treatment Of Cervical Spondylotic Myelopathy Accompanied Unsteady Ceruical Uertebra

Objective: The comparison of curative effect about unilateral open door expansive laminoplasty of one side with internal fixation with lateral masses and unilateral open door expansive laminoplasty of one side in treatment of cervical spondylotic myelopathy accompanyed unsteady ceruical uertebra.Methods: To analyze retrospectively 48 patients with cervical spondylotic myelopathy accompanyed unsteady ceruical uertebra accepted from June 2005 to August 2006.They were divided two groups randomly.There are 26 patients in group one whose type is unilateral open door expansive laminoplasty of one side with internal fixation with lateral masses, meanwell, there are 22patients in group two with the other type.Then to evaluate the time of operation, the volume of blood,the functional recovery of nerves(JOA score) and the measurement of the physiological curve of ceruical uertebra. Results: The mean follow-up time is 13 monthes, and all patient can improve with different degree.The opration time of group one is 143.08±4.41 minutes, and the volume of blood is 383.08±43.91 ml;The time of group two is 143.63±12.61 minutes ,and the volume of blood is 329.55±50.61ml,there is no statistical significance.The JOA score of group one at preoprative, two weeks after opration and now is 9.42±0.42, 12.38±0.42 and 15.04±0.36;The score of group two is 9.45±0.46, 10.55±0.5 and 13.59±0.38,it has statistical significance .The effective power of operation at group one is 92%, and the good rate is81%;The group two is 86% and 64%,the effective power betweenthe two group has not statistical significance,the good rate has statistical significance.There is no statistical significance about measure of the curve of ceruecal uertebra at preoprative and post oprative in group one ,but it has statistical significance in group two.Conclusions: There is no more time of opration and volumeof blood by unilateral open door expansive laminoplasty of one side with internal fixation with lateral masses screw,but can strengthen the stability of ceruical uertebra at the time of thorough depress through canalis spinalis.It is a simple,safe and effective method to the patient of cervical spondylotic myelopathy accompanyed unsteady ceruical uertebra.
