Objective We investigated the feature and clinical meaning of the plasma homocysteine (Hcy) levels of the pregnant women who delivered their babies at 28~34 weeks and the relative factors.Methods In a control-case study, we investigated the plasma hcy levels of 60 pregnant women who delivered their babies at 28~34 weeks( the case group) and 30 normal pregnant women with close gestational age(the control group ) using HPLC-FD. Linear correlation analysis was used to analyze the feature and clinical meaning of the plasma hcy levels of the preterm birth. Results The 95% reference values of hcy levels of the control group was 5.96±2.54 umol/L.The hcy levels of the two groups were affected directed by gestation age and folate level, especially inversely correlated with the latter (the case group γ =-0.844, P=0.000; the control group γ =-0.658, P=0.000),but no direct relation with the age. The hcy levels of the case group(7.56 ± 2.97 umol/L) was significantly higher than the control group (5. 96±2. 54 umol/L), P=0. 013. The raise of the hcy levels of the case group didn't elevate the risk of preterm birth if we got rid of the confound factors. The hcy levels of the two groups were inversely correlated with the neonate birth weight (the case group γ =-0.696, P<0.01; the control group v =-0.733, P<0.01); A elevated plasma hcy level could increased the risk of deliverying SGA; The possibility of deliverying SGA was significantly increased when the hcy levels of the case groups ≥ 11.0 umol/L (x~2=8.597, P =0.003 ), but other complications of preterm infants didn't increased. Conclusion The plasma hcy levels of the pregnant women delivered their babies at 28~34 weeks were affected by physiology , diet and other factors , and changed according to the gestational age and folate levels. The raise of the hcy levels of the case group didn't elevate the risk of preterm birth, but increased the possibility of SGA infant.
