Objective To make a retrospective analysis about the cases of lingual root carcinoma, and explore the surgery operation of the squamous cell carcinoma of lingual root , the applied extend of different approach and functional reconstruction.Methods From March 1994 to March 2004, a retrospective analysis was performed with 31 cases of lingual root carcinoma in the Qilu Hospital of Shandong University, including TNM stagings, histopathology stagings, oparative approach, functional reconstruction and so on. All cases were squamous cell carcinoma, and neck dissection were performed. The tumor was removed by the surgical approaches to oral cavity, latero-pharynx, regiones hyoidea, mandibular swing approach, the function was reconstructed with sternohyoideus fascia muscularis flap, pectoralis major myocutaneous flap and so on.Results Among the 31 cases of lingual root carcinoma, â…  stage: 7 cases; â…¡ stage: 2 cases; â…¢stage: 14 cases; â…£stage: 8 cases. Major operative approach were latero-pharynx and mandibular swing approach , the function was reconstructed with sternohyoideus fascia muscularis flap and pectoralis major myocutaneous flap. All patients'swallow function were recovered successfully after post-operative training, and all patients' linguistic function were recovered successfully expect three cases with the trachea cannula. 3-year and 5-year survival rate is 61.3% (19/31) and 51.9% (14/27) respectively.Conclusions Perfect preoperative examination and the exact assessment about the tumor extent is very important to choose appropriate surgical methods and resect the tumor completely. The exact surgerical approaches and repair methods can be used to remove the tumor totally, to reconstruct the function successfully, and to improve the life quality efficiently. Major operative approach were latero-pharynx and mandibular swing approach, the function was reconstructed with stemohyoideus fascia muscularis flap and pectoralis major myocutaneous flap.
