BackgroundsIntracranial aneurysm is one kind of stroke with high disability rate and mortality. About 80 percent Subarachnoid hemorrhage (SAH) cases are associated with ruptured Intracranial aneurysms. Furthermore, Intracranial aneurysms are always asymptomatic until the SAH occurs, which might make the mortality as high as 50 percent. Generally, Intracranial aneurysm occurs in up to 2.3 percent of the population, taking the third place in the stroke diseases. Therefore, knowing the differences about the risk factors and the speciality of Intracranial aneurysm in different population provides nurses with expertise about this disease, then nurses could spread healthy information to the patients to help them get a nice prognosis. By far, many researches on the risk factors for Intracranial aneurysm have been down in abroad, but the scientists still have not come into a conclusion. Meanwhile, there is no research on the differences of risk factors and aneurysm condition in different population. Consequently, such a research would be helpful in nurses' daily work. It could enrich the content ofhealth education with which nurses could instruct the patients to receive therapy efficiently. As the medical science and technology develops, not only the diagnosis skills but also the therapies have been improved. What's more, operation has been applied more and more often in the treatment of Intracranial aneurysm patients. Patients with the highly risky, large wounded operation might suffer from severe complications, which would make a bad prognosis and prolong the time in hospital. This phenomenon has been proved by many scientists. A series of researches showed that patients' condition before the operation has significant influence on prognosis. In nursing area, there is no research on showing the relationship between the factors and prognosis, though nurses' concernment about it could help them cooperate with doctor well and make the patients recover soon.ObjectivesThe medical records of 201 Intracranial aneurysm patients who were admitted in the 2th affiliated hospital from March 2004 to August 2006 were recorded and analyzed to investigate the relationship between patients' condition and their prognosis. With the analyzed result we could also find out the differences of risk factors and aneurysm condition in different population. All these findings could be used in nurses' daily job to make them work efficiently.MethodsBefore the research, we make a data-collecting table which includes potential related factors for aneurysm occurrence, development and prognosis, by referring to articles both from abroad and inland. Then, according to the standard for the test group, 201 patients' medical records were written down byone researcher including demographic data, living habits, history of illness, physiological and pathological test, aneurysm condition. In order to find out the possible related factors of prognosis and investigate the differences of risk factors and aneurysm condition in different population, spss13.0 software was applied for data analysis.ResultResults of X2 test, t test and logistic regression showed: Hypertension, management of blood pressure, Hunt-hess classification, serum glucose level would influence the prognosis of Intracranial aneurysm (P<0.05) .Especially, serum glucose level has significant relationship with prognosis (P<0.01) . By analyzing the risk factors in the 201 patients, we got the conclusion that the morbidity reaches its peak at 40-60 years old population, and Intracranial aneurysm occurs in women more than men. Among the causes of this disease, the patients with no apparent cause took up the largest percentage, the following causes are labour, waking up in the morning, injury and agitation. As one of the risk factors, Hyperlipidemia is the most popular factor, the ordinal following factors are smoking, Hypertension, tipple and familial history of SAH. In addition, the risk factors and aneurysm condition have significant difference in different population. Smoking and tipple rates are higher in men than women (P<0.01) . Hypercholesterolemia and multiple aneurysm occur more often in women (P<0.05) . The location of aneurysm also differs between men and women (P<0.01) . On the other hand, the 40-60 years old patients have higher LDL level and smoking rate than other patients (P<0.05) .Hypertension, high serum glucose level, Hypercholesterolemia are the most popular risk factors in patients older than 60 years old.Conclusions1. Hypertension, management of blood pressure , Hunt-hess classification and serum glucose level have relationship with prognosis after the Intracranial aneurysm operation. Nurses should pay attention to the blood pressure and serum glucose level before operation.2. The morbidity of Intracranial aneurysm is highest in 40-60 years old patients. As a whole, female patients are more than male patients, especially after 40years old.3. Most patients have no disease causes. But among those patients who have causes, labour is the most popular cause, and the following ones are waking up in morning, injury and agitation.4. In all of the 201 patients, Hypercholesterolemia is the most common risk factor, smoking takes the second place, then Hypertension, tipple and familial history of SAH are less frequent in the patients.5. Smoking, tipple, location of aneurysm, serum cholesterol level, occurrence of multiple aneurysm have significant difference between female and male patients.6. Hypertension, smoking, serum glucose level, serum LDL level and serum cholesterol level have significant difference among patients with different ages.
