Objective: To evaluate the clinical significance of the differentiation diagnosis of SAAG,blood plasma glucose,ascites/blood plasma glucose ,ADA,LDH ,CEA,CA-50,CA-199,AFP,VEGF and MMP-2 in ascites, between malignancy-related and nonmalignant ascites ,so as to raise the diagnosis accuracy, by the incorporating ascites measurement.Methods: Total 70 ascites samples were collected,there were 30 patients with cirrhosis ascites(including 4 SBP),11 with tuberculous ascites,3 with cardiac ascites and 26 with tumorous ascites .The review research method was adopted. Index signs ,above-mentioned in ascites from patients with clear pathogeny,was measured. By statistics analysis,the clinical significance of the differentiation diagnosis between malignancy-related and nonmalignant ascites was observed. The serum(syrup) and ascites ALB,GLU,LDH was measured by a automatic biochemistry analysis apparatus , CEA,CA-50,CA-19 9,AFP,ADA was measured by a radio immuno assay, VEGF and MMP-2 was measured by an enzyme-linked immunosorbent assay.Results: among them.SAAG≥11.0g/L in all patients with portal hypertension ascites ,while SAAG<11.0g/L in all ones with non- portal hypertension ascites. The sensitivity,specificity and diagnostic efficiency of ADA,blood plasma glucose,ascites/blood plasma glucose for the diagnosis of tuberculous ascites were: 81.8%,90.9%,72.7%;86.4%,93.2%,94.9%;85.7%,92.8%,91.4%,respectively, and a combined diagnostic efficiency showed better than a single determination was: 90.0%. The sensitivity,specificity and diagnostic efficiency of CEA,CA-50,CA-199,AFP for the diagnosis of tumorous ascites were: 53.8%,50.0%,38.5%,11.5%; 79.5%,86.4%,83.7%,100%;70%,72.9%,70.0%,67.1%, respectively, that of CEA+CA-50+CA-19 9 was: 34.6%,93.2%,71.4%. The sensitivity,specificity and diagnostic efficiency of VEGF+MMP-2 for the diagnosis of tumorous ascites was: 53.8%,100%,82.9%.The sensitivity of VEGF was greater,while the specificity was poor.
