Capacities Assessment Of Response Public Health Emergencies In The Field Of County Disease Prevention And Control Organs In Luoyang City

Research objective County organs for disease prevention and control as the base of response public health emergencies, its emergency response capacities directly influence the construction and perfection of public health emergencies response systems in our country. This article collects public health emergencies responded by 15 county organs in Luoyang city during 5 years, by analyzing epidemic characters of these cases, assessing every factors of response and management capacities in the field and conducting questionnaires to personnel finds out the strongpoint and shortage in responding public health emergencies, gives to pertinent improving measures and provides basis for fully improving of response and management public health emergencies capacities in the field of county organs for disease prevention and control in Luoyang.Research method Take survey method of review collect all field disposal information of public health emergencies outbreak on infectious disease or initial report with unclear cause disease during 2000 to 2004 total 15 county organs in Luoyang including 8 counties, 1 city and 6 districts, fill out analysis form and analyze their epidemic characters. Take Delphi method construct epidemic emergencies on site disposal assessing guideline systems of Luoyang city. Take stratified analysis confirm every guideline' s scale and assessemergency responding capacities in the field. Take questionnaires and interview survey county organs personnel about general information, understand and opinion on local response and management capacities in the field and seized of specialty knowledge.Research result Since 5 years, public health emergencies have average 16 cases annually in Luoyang city. Of which, breathing canal communicable diseases much happened as 77.5 percent. It is no clear difference in different districts. Only very few districts are much discovered. And after SARS for the increasing of sensitivity for epidemic disease of disease prevention and control organizations, these cases clearly increased. Country and school are the incident outbreak places by 75 percent and 52.5 percent each. The youths are the incident outbreak herd. The outbreak reason is basal sanitation and disease prevention has defectives in one hand and response capacity of public health emergencies is lack in the other hand.The comprehensive score of 76 cases of public health emergencies are: the lowest 57.0240, the highest 96.7130, average 78.6671, the middle 78.8430. The comprehensive score during 5 years are no distinct differences and the cases are clearly discovered in few districts. Within the every steps of emergencies response, surveillance information of epidemic situation is mostly from routine report mode by 82.89%. The interval between initial case and initial report lasts longer. The sensitivity is not well. Taking 24 hours as standard, the response passed rate is 73.68%. During field survey, verifying diagnosis generally adopts clinic diagnosis by 55.3%. The supporting rate of laboratory to epidemic diseases survey is lower just as 35.5%. In aspects of confirming outbreak, area distribution, time distribution and population distribution and risk factors, it has some level. But statistics guidelines applied just as 30.2%. During fielddisposal, the measures for infectious source, transmission route and susceptible population are basically correct. Health training and psychological intervention developed by 46.05%. The timeliness of response is well and passed rate is 84. 21%. The effectively controlling rate is 77.63%.The survey to the stuff of county disease prevention and control organs in Luoyang shows: at present county disease prevention and control centers have some public health emergencies response capacities but not form a perfect system. Human resources team construction and surveillance system construction are the keystones. And the emergency response materials should still be routinely hived. If response capacity would be improved in short order, there must have enough financial assuring.Conclusion and suggestion By survey and analysis, the conclusion in this article is: county disease prevention and control organs in Luoyang have some public health emergencies field response capacities but still have many questions mainly represent: surveillance and alarm system is not perfect. Laboratory supporting to epidemic diseases disposal is not enough. The stuff has still distance to certain specialty knowledge and level and their knowledge structure urgently need update. If want improving emergency response capacity quickly, firstly enough financial support should be ensured, surveillance system construction should be strengthened, surveillance capacity should be improved, especially by field-based training to improve personnel quality continuously.
