Objective: To assess the role of leptin in the pathogenesis of coronary heart disease (CHD) in men and the relationship among serum leptin, insulin resistance and coronary heart disease (CHD) in men. Methods: Fasting serum leptin (Lep), fasting insulin (FIN), blood pressure, body mass index (BMI), fasting plasma glucose (FPG), total cholesterol (TC), triglyceride (TG), HDL cholesterol and LDL cholesterol were determined in 137 men with coronary heart disease (CHD) and 203 males as controls..Serum leptin concentration were measured by enzyme-linked immunosorbant assays (ELISA.). Homeostasis model assessment was applied to assess the status of insulin resistance (HOMA-IR). The correlation between leptin and other parameters were analyzed. Results: 1) There were no statistical differences in age, BMI, blood pressure and FPG among the two groups (p>0.05). Lep, FIN, HOMA-IR and TG levels were significantly higher in CHD than those in control subjects (p<0.01), especially in Lep levels (p<0.001); The multiple correlation analysis showed that leptin levels were signifinantly correlated with BMI, TC, TG, FPG, SBP,DBP and FIN (r>0.4, p<0.01) and increased with the narrow aggravation of coronary artery (p<0.05). 2) FIN levels were signifinantly correlated with Lep, BMI, TG and TC (r> 0.4, p<0.01), frequency of insulin resistance in grade I , grade II, grade III was 25.0%, 34.7%, 40.0%, Compared to the control, OR was 1.4,2.2 and 2,8. 3) Logistic regression analysis showed the risk factor for male CHD were Age, BMI, FPG, TG and Lep. Conclusion: It showed thatleptin remained significantly associated with CHD after adjustment for age, BMI, blood pressure and FPG, Increased leptin levels might be a risk factor for male CHD. High leptin levels were significantly associated with insulin resistance,and they may play an role in coronary heart disease.
