Interstitial Cystitis ( Simple as IC ) is a chronic sterile inflammatory disease of the bladder with unknown etiology characterized by urinary frequency, urgency, nocturia and suprapubic pain. The etiology of interstitial cystitis remains obscure. Hunner of The Johns Hopkins Hospital in Baltimore produced the most well-known early report on IC in 1915, describing erythe matous bladder lesions characteristic of end stage disease in 5 patients. In 1987, the NIDDK formed a consensus definition of IC. The guideline has not served its purpose since it was basically a concept of exclusions and based on evidence. The aim of drawing up these guidelines was to have an international standard to enable a comparison of patients in different geographical areas. Today, some 17 years after these research guidelines were first drawn up, the original aim has not been fulfilled. It has been estimated that if the guidelines are strictly followed, around 60% of patients will fail to be diagnosed.Most international urologists introduced the potassium sensitivity test from early of 21st century. This test has been shown to be positive in patients with IC. Pelvic pain and Urgency/Frequency patient sympotomscale have been included as an optional test as consensus in 2004.A new consensus has been accepted by many urologists.This article includes two parts. First, the study comprised 11 controls and 11 patients, both are women. All patients were identified with NIDDK criteria, and were performed with the potassium sensitivity test and pelvic pain, urgency/frequency patient symptom scale. Controls and patients were assessed with survival urine volume and maximum bladder capacity. All women had undergone cystoscopy under local anesthesia. Above evidences show that patients' the potassium sensitivity test and pelvic pain, urgency/frequency patient symptom scale are different with controls' . The new diagnosis criteria is better to NIDDK criteria.The second part has studied bladder biopsies from 22 cases of interstitial cystitis and control groups. Indirect immunofluorescence was performed on the biopsies using murine MoAbs to human HLA class II molecules HLA—DR. In normal bladder HLA class II expression was seldom and cymphocytes restricted to submucosal cells. In all cases of interstitial cystitis showed surface expression of HLA—DR molecules in the epithelium and lymphocytes cells in the submucose.
