Objective To analyze brain stem and acoustic neuroma inmicrosurgeryMethods 145patients with acustic neuroma were classified into0-4 stages according to the degree of tumor compress to brain stem byradiological features and operation record 0: acoustic neuroma havenot touch with lateral margin of brain stem, aqueduct and fourthventricle situation middle, aqueduct displace angle 0 degree(graph1). I:brain stem of lateral margin part depression, aqueduct and fourthventricle situation middle or slightly displace opposite side. aqueductdisplace angle less than 5 degree (graph2). II brain stem of lateralmargin, aqueduct and fourth ventricle obviously comma change toopposite side, aqueduct displace angle 5-25 degree (graph3). III brainstem of lateral margin, aqueduct and fourth ventricle obviously commachange to opposite side and stenosis, obliteration and hydrocephalus,aqueduct displace angle more than 25 degree (graph4). (aqueductfelling cut and aqueduct displace maximum line and medianlineincluded angle setting to aqueduct mode according to grading).Result 84.8% of tumor were total removed, The facial nerveswere anatomic preserved by 70.3%, acoustic neuroma anatomicpreserved by 14.5%, 2patients death. In 0-I, II and III groups tumorincidence rate, facial nerves preserved and midrange above facial palsyafter operation have not signigicance difference (P<0.05).
